Providing healthcare services to residents in rural areas has presented increased challenges that include access and cost. For your Capstone Project, you will be using a case study that focuses on Pocahontas Memorial Hospital located in Pocahontas County, West Virginia.

Pocahontas Memorial Hospital (PMH) would like to offer a comprehensive satellite health clinic within Pocahontas County. You have been contracted to prepare a report for the PMH Board of Directors to guide them in determining the feasibility of opening a satellite clinic as part of a strategic initiative for their annual strategic management planning process.

The behavior I have chosen is that of my exercise regime. Currently, I work full-time, homeschool my daughter, and attend online schooling. Due to this, I do not incorporate a lot of physical activity into my daily schedule and considering baseline data this shows my level of exercise is almost nonexistent. By choosing adding exercise to my daily routine as my topic for the self-management project I hope to become healthier. To begin this project, I will need to evaluate my bodies current abilities in regard to exercise and stamina. Determining the best approach at exercise in the amount of time allotted is imperative as well.  For each day of the project a record of which exercises completed, in addition to the amount of time dedicated will be logged. By keeping track of the different exercises and time, I should be able to modify by exercise regime to one that benefits my body and schedule, revising if deemed necessary. By keeping an observable and measurable record, I will be forced to hold myself accountable.

Assessment Instructions

In Assessments 1–4, you completed four parts of a request for proposal (RFP) from a nearby school district that is seeking individuals or groups to design a training and professional development inservice day about the brain and mind from a biopsychological perspective for their educators. In addition to providing professional development inservice for educators, the school administrators have also asked you to prepare an evaluation of intelligence testing and the value of introducing biological psychology to educators, social workers, and other professions. This is Part 5 of the RPF.

Using the Assessment 5 Proposal Template (linked in the Resources), complete the following three sections for this assessment. Use the Capella library to research the material and support your answers.

  1. Conduct an Internet search to locate an IQ test. You wish to use the intelligence quiz on the Mensa Web site.
  2. Complete the IQ test. During the test, have pen and paper ready to jot down notes and observations about the test.

Gloria has recently graduated college and started a new job. Her coworkers seem very nice, and Gloria has gotten to know Lakeisha and Reggie the best.

Reggie is a friendly older man who often talks about his plans for retirement in five years. He likes to make people laugh but often jokes about his struggles to learn new programs and technology.

Gloria and Lakeisha have been assigned to work on a project together. Lakeisha is very organized and has prepared a list of to-do items along with due dates. She even color-coded the list to indicate which partner will do each item. Gloria, who is more laid-back, feels a bit uncomfortable with this micromanaging from Lakeisha, who has only been at the job a few months longer than she has. Gloria wonders if Lakeisha thinks she is either lazy or stupid, and she does not look forward to working on this project. In the past, Gloria attempted to communicate her feelings with a co-worker; however, her coworker told Gloria that she needed to put her feelings aside and just do the work assigned to her.  As Gloria ponders having a conversation with Lakeisha, she starts to feel overwhelmed and thinks she might have to ask to be removed from this project.

Still, she and Lakeisha have connected because they are both single mothers of teenagers. Gloria confides in Lakeisha about her 16-year-old son who has started making poor choices and is currently grounded for sneaking out of the house and getting drunk at a party the night before a big exam.

The company recently informed the team that there will be a compliance test on new safety policies, which employees will need to pass in order to keep their jobs. The company has provided materials to study as well as optional practice exams. Gloria and Lakeisha have signed up to take the first practice test. Reggie, however, jokes that he will probably wait until the night before the test to read the material. As the date of the test approaches, Reggie becomes increasingly nervous. He makes frequent comments about how hard it is for him to learn a different way of doing the jobs that he has been doing for years, joking that “you can’t teach an old man new tricks.”

End-of-life care becomes an issue at some point for elderly clients. Even with the emergence of palliative care programs and hospice programs, the majority of elderly people do not die in their own home as is their preference. What are the reasons for this trend? Discuss what you can do as a nurse to support your clients in end-of-life care and in supporting their desires. Support your response with evidence-based literature.

Most people consider home deaths as an outcome of high quality palliative care. The view of the home as the optimal place to die has been challenged, with the establishment of palliative care in hospitals, nursing homes, hospices and other in-patient facilities. Most patients wants die with the presence of their loved ones and be able to enjoy the last few moments with them. For many, not being able to die at home could therefore be seen as a failure in the care given to these patients, even if the patient was actually admitted to e.g. a hospital or hospice for perfectly valid reasons and in accordance with the patient’s wishes. There are many more reasons why most elderly patients prefer to die at home. With the emergence of home health care team, it encourages medical care to be continued at home while enhancing patient’s quality of life with their loved one. According to the article “Where do elderly patient’s prefer to die”, failure to provided death at home was mostly due to a prolonged terminal phase leading to emotional and physical exhaustion in the patient or the family (Groth-Juncker A., 2015).

In accordance to the American Nurses Association(ANA) statement position, it is the nurses duty to provide comprehensive and compassionate end-of-life care . Nurses are obligated to provide care that includes the promotion of comfort, relief of pain and other symptoms, and support for patients, families, and others close to the patient. Throughout this position statement, the term “family” includes those linked by biology or affection; family is whomever the patient says it is (ANA, 2016). In addition, it is also the nurses responsibility to discuss with the physician the patient’s wishes and incorporate social worker and pastoral care.

I currently work in Medical Surgical Oncology unit where I deal with death and dying quite often. There are many cancer patients who wants to die at home rather than at the hospitals, reason being they want to spend their last few days with their family. Most of these patients were offered hospice care, palliative or home health. Although the patient choses one of these care continuum care, it is possible to have it continued at home and they don’t have to stay at the hospital unless they do not have any family, home or anyone whom they can spend their last few moments with.

1-What are the reasons for elderly patients not dying at home as they desire?

Poor communication leading to unwanted care, increase in repeat hospitalizations, increase in late transitions, increase in ICU usage, fee-for-service care attracting financial gain, family members wanting interventions, medicare rules not allowing reimbursement for both skilled nursing facility care and hospice care.

2-Interventions/ activities/ referrals to support your clients in end-of-life care and in supporting their desires

Physicians, patients and the families should discuss end of life option and develop a plan early, Patients should complete an advanced directive and living will clearly stating their end of life choices, Patients should assign a health surrogate that is aware of their wishes, health care facilities need reform, medicare needs reform, patients need earlier referrals to hospice.

The aspect that I found most interesting in the reading is the polarity between the older and the newer nursing theories. The older nursing theories are associated with taxonomies and quantitative measures, whereas the newer more holistic nursing theories are more qualitative measures and softer phenomena (Shelly, J., 2006). The revolution in the approach of how we think about nursing and nursing interventions in association to ethical decision making relates to a belief that nursing as a profession is both  a science and an art-based profession, and the the new theories expand upon the concept. The other aspect that is interesting to me is using world views to challenge contemporary ethos by using philosophical clarity, religious sensitivity, and proportionate level of care for the sick and others who may need other forms of physiological and psychological interventions and/or wellbeing.

As medicine is traditionally considered a healing profession, and modern medicine claims legitimacy to heal through its scientific approach to medicine (Starr, P., 1982), the marriage of science and medicine has generally empowered nurses and physicians to intervene actively in the course of disease, to effect cures, to prevent illness, and to eradicate disease Hauerwas, S. (1990). In the wake of such success, nurses and physicians trained as biomedical scientists, have focused on the diagnosis, treatment, and prevention of disease. In the process, cure, not care, became the primary purpose of medicine; as nurses and physician’s role have transformed to the “curer of disease” rather than “healer of the sick (Starr, P. (1982). Thus, healing in a holistic sense has faded from medical attention and is rarely discussed in the medical literature.

The polarity between the older and newer nursing theories is the aspect which I have found to be the most interesting during my reading. From my reading, I have found that the older theories are a quantitative while, the newer ones are of qualitative measures.The revolution in the approach of how we consider nursing and nursing intercessions in relationship to basic moral leadership identifies with a conviction that nursing as a calling is both a science and a craftsmanship based calling, and the new speculations develop the idea (Puchalski, Blatt, Kogan, & Butler, 2014).The other perspective that is intriguing to me is utilizing world perspectives to challenge contemporary ethos by utilizing philosophical clearness, religious affectability, and proportionate level of watch over the wiped out and other people who may require different types of physiological and mental intercessions and additionally prosperity.
