An Ohio State University study found that fewer than a third of nurses surveyed had health care mentors that were available to help them learn about evidenced-based practices. It is imperative that new nurses need to be partnered with model mentors that are dedicated to seeking out current research. Established nurse leaders need to make more time to seek out current research and communicate when change needs to be made. A way to change this would be to have nurses review a procedure or current concern each quarter for their floor and see how they are rating.  Complacency tends to happen when there is no plan.

Another barrier that was identified in this same study was that nurses may stick to the tradition or previous teaching that they received when they were originally educated. New nurses who have received more up-to-date evidenced based training in school, may be up against nursing tradition when they come to the workplace.  These nurses may not be allowed or encouraged to implement this new evidence based practice and be orientated to the organization’s old policies and procedures (Nursing Times, 2012). A great way to tackle this is to survey new nurses throughout orientation and ask them if they were taught with different technique.  Once discrepancies are identified, the nurse manager could begin looking into the EBP for that concern.

One of the historical experiments that was instrumental in shaping the future of ethics and regulations in research is the famously known Tuskegee experiment which was conducted without the participants giving their informed consent to participate in the study and the researchers did not inform the participants the risks and benefits of participation and the reason behind conducting the research. The Tuskegee Institute together with the Public Health Service started the study in 1932 with the aim of finding out the history of syphilis among the black men hoping that they would start treatment programs. 600 men participated in the study. 399 of the men had syphilis while 201 men did not have syphilis. The researchers were lying to the men that they were treating them for having bad blood which was a term that was used to refer to long term illnesses associated with fatigue, anemia and syphilis. This study was supposed to run for 6 months but it continued up to 40 years (Center for Disease Control, 2017).

The participants were compensated with free medical examinations, meals and burial insurance. What went wrong with the study was discovered 40 years later after a press story lead to public outcry and a review of the study was done by a panel of 9 members. The panel found out that there was no evidence to show that the men had been informed about the study and its main purpose and no facts were provided to them at all. They agreed to be examined and treated freely. Still, in 1947, after penicillin was found to be a cure for syphilis, the men were never treated using penicillin neither were they given the chance to quit from participating in the study. The study ended in 1972 and in 1973 a lawsuit was filed on behalf of the involved participants and the government promised that it would give lifetime medical benefits to all those who survived, their wives and offspring’s and they would also offer burial services to them. On 16th May 1997, the Presidential apology was given by President Clinton on behalf of the nation and in 2001 the Presidential Council on Bioethics was established. The last person to receive these benefits was a widow who died on 27th January 2009 (“Tuskegee Syphilis Study and Its Implications for the 21st Century,” 2015).

Gallagher (2014) suggests that to make the study ethical and valid, the participants were supposed to be informed about the objective behind their required participation in the study and in case they never wanted to participate, they should not be coerced into the study. Secondly, the participants were supposed to be treated using penicillin which was discovered to cure syphilis in 1947 and this would have reduced the deaths. More so, there were no records on the number of deaths that resulted from the study. The researchers were supposed to have followed up on the participants and keep records whether they died from syphilis. In a nutshell, the study violated the rights of the participants by keeping them on the dark on what was happening.

One aspect of the topic readings that I find the most interesting is the role of spirituality and religion in health care. I find this topic to be the most interesting, because I feel that it is the most important area to understand in order to deliver holistic care. Nursing care has transformed over the years with a large emphasis on delivering holistic care. Holistic care involves a comprehensive care model that incorporates a patient’s physical medical condition, emotions, spirituality, religion, ethnicity, cognitive status, economic status, environment, genetics, etc. In order to delivery holistic care spiritually must be addressed, thus why the role of spirituality in health care is important and interesting to me.

According to the article, The Role of Spirituality in Health Care, “Spiritual or compassionate care involves serving the whole person—the physical, emotional, social, and spiritual” (Puchalski, 2001). Compassionate care is very important to produce positive patient outcomes. Spiritually and religion may guide one’s worldviews, influencing all their decisions they make. It is important for health care providers to incorporate spirituality into health care to promote positive relationships and health and wellness. When health care providers ignore spiritual and religious beliefs, patient’s may turn away from care delivered and refrain from participating in health promoting measures/interventions. When health care providers can incorporate patient’s spiritual values and beliefs, it can foster health and wellbeing. “Compassionate care calls physicians to walk with people in the midst of their pain, to be partners with patients rather than experts dictating information to them” (Puchalski, 2001).

This topic has a strong correlation with disease and healing. Spirituality is a strong guiding point for healing and how individual’s view disease and illness. Some individuals may turn solely towards religious and spiritual beliefs for healing rather than incorporating western medicine. Some patients may choose to incorporate both to produce the best outcomes. Regardless of the patient’s views on healing and disease, it is important that health care providers take a stance in understanding each individual’s viewpoints. Christina Puchalski stated in the article, “Cure is not possible for many illnesses, but I firmly believe that there is always room for healing. Healing can be experienced as acceptance of illness and peace with one’s life. This healing, I believe, is at its core spiritual” (Puchalski, 2001). It is essential that all health care providers complete an extensive spiritual history on all patients to gain a better understanding of their expectations of care, healing, disease/illness in order to delivery appropriate and effective care. “A spiritual history helps physicians recognize when cases need to be referred to chaplains. It opens the door to conversation about values and beliefs, uncovers coping mechanisms and support systems, reveals positive and negative spiritual coping, and provides an opportunity for compassionate care” (Puchalski, 2001).
