Psychological Treatment Plan2

Psychological Treatment Plan2

(Psychological Treatment Plan2)

It is recommended that students review the e-book The Complete Adult Psychotherapy Treatment Planner (Jongsma, Peterson, & Bruce, 2014) for additional assistance in completing this assignment.  Clinical and counseling psychologists utilize treatment plans to document a client’s progress toward short- and long-term goals. The content within psychological treatment plans varies depending on the clinical setting.

The clinician’s theoretical orientation, evidenced-based practices, and the client’s needs are taken into account when developing and implementing a treatment plan. Typically, the client’s presenting problem(s), behaviorally defined symptom(s), goals, objectives, and interventions determined by the clinician are included within a treatment plan.  To understand the treatment planning process, students will assume the role of a clinical or counseling psychologist and develop a comprehensive treatment plan based on the same case study utilized for the Psychiatric Diagnosis assignment in PSY645. A minimum of five peer-reviewed resources must be used to support the recommendations made within the plan. The Psychological Treatment Plan must include the headings and content outlined below.  Behaviorally Defined Symptoms

•Define the client’s presenting problem(s) and provide a diagnostic impression.

•Identify how the problem(s) is/are evidenced in the client’s behavior.

•List the client’s cognitive and behavioral symptoms.  Long-Term Goal

•Generate a long-term treatment goal that represents the desired outcome for the client. ◦This goal should be broad and does not need to be measureable.   Short-Term Objectives

•Generate a minimum of three short-term objectives for attaining the long-term goal. ◦Each objective should be stated in behaviorally measureable language. Subjective or vague objectives are not acceptable. For example, it should be stated that the objective will be accomplished by a specific date or that a specific symptom will be reduced by a certain percentage.   Interventions •Identify at least one intervention for achieving each of the short-term objectives.

•Compare a minimum of three evidence-based theoretical orientations from which appropriate interventions can be selected for the client.

•Explain the connection between the theoretical orientation and corresponding intervention selected.

•Provide a rationale for the integration of multiple theoretical orientations within this treatment plan.

•Identify two to three treatment modalities (e.g., individual, couple, family, group, etc.) that would be appropriate for use with the client.  It is a best practice to include outside providers (e.g., psychiatrists, medical doctors, nutritionists, social workers, holistic practitioners, etc.) in the intervention planning process to build a support network that will assist the client in the achievement of treatment goals.  Evaluation

•List the anticipated outcomes of each proposed treatment intervention based on scholarly literature. ◦Be sure to take into account the individual’s strengths, weaknesses, external stressors, and cultural factors (e.g., gender, age, disability, race, ethnicity, religion, sexual orientation, socioeconomic status, etc.) in the evaluation.

•Provide an assessment of the efficacy of evidence-based intervention options.  Ethics

•Analyze and describe potential ethical dilemmas that may arise while implementing this treatment plan.

•Cite specific ethical principles and any applicable law(s) for resolving the ethical dilemma(s).  The Psychological Treatment Plan

•Must be 8 to 10 double-spaced pages in length (not including title and references pages) and formatted according to APA style as outlined in the Ashford Writing Center (Links to an external site.)Links to an external site..

•Must include a separate title page with the following: ◦Title of paper ◦Student’s name ◦Course name and number ◦Instructor’s name ◦Date submitted

•Must use at least five peer-reviewed sources in addition to the course text.

•Must document all sources in APA style as outlined in the Ashford Writing Center.

•Must include a separate references page that is formatted according to APA style as outlined in the Ashford Writing Center.

Assignment:(To Be/Not To Be Civil?)

Now that you have discovered what incivility is, let’s review therapeutic communication techniques by viewing the video link on the study.com website:

http://study.com/academy/lesson/therapeutic-communication-definition-goals-types-principles.html

Choose one scholarly, peer-reviewed   on incivility in the nursing workplace or the classroom. Write a one to two-page summary, using APA 6th edition writing mechanics, of your article. Add three of your personal thoughts or insights (in a few sentences) at the end of your summary. It could be something you have experienced at school or work or saw happen in a classroom or work situation. Finally, give some thought as to how you would manage this situation (remember therapeutic communication techniques): Would you feel comfortable reporting these uncivil actions?; who would you report it to?; if the situation happened to you, would you use productive confrontation to address the person who was uncivil?; if you have been uncivil, what are your plans to change your behaviors?; and what therapeutic communication techniques would be best to use in this situation? You can add anything else you might like to address concerning incivility. Please in-text cite your reference/s and add them to a reference list at the end of the paper in APA 6th format.

Hopefully, you will be able to help advocate for courteous behaviors from here on out in your career. It is critical that we all RESPECT each other!

Developing Evidence-Based Practice

Developing an Evidence-Based Practice (EBP) Project Question

(Developing Evidence-Based Practice)

Consider issues related to your nursing practice. What stands out to you about your specialty area? What questions or concerns do you think are especially in need of attention? Why? How would you most like to contribute to the knowledge base of your profession?

The EBP Project provides a significant opportunity for furthering the nursing profession. Formulating a specific, applicable question is a necessary first step for developing your project. The question defines the focus of the EBP Project and guides your selection of research methods and tools. Think about aspects of your specialty area and begin to formulate a question that relates to your nursing practice.

The primary purpose of this Discussion is to provide a forum in which you and your colleagues can support each other in cultivating a practice-related question.

To prepare:

  • Develop      your EBP Project question using the PICO (patient group, issue      or intervention, comparison, outcome) method as described in Chapter 2      of Clinical Research for the Doctor of Nursing Practice (p.      29) and other resources. Depending on your specialty area, you may modify      the PICO method as necessary to be appropriate for your target population.
  • Begin      to complete the “Literature Review Matrix,” reviewing at least four to six      research articles relevant to your specialty area and      your EBP Project question. Note any areas of difficulty that you      encounter, as well as any strategies or insights that might be beneficial      to others.
  • Reflect      on the information presented in the McCurry, Revell, and Roy article      listed in the Learning Resources.
  • What      questions or concerns, if any, do you have about how to develop your      project question and/or expand your expertise in your specialty area?

By tomorrow 08/28/18 10pm write a minimum of 550 words in APA format with at least 3 scholarly references from the list of required readings below. Answer the questions as numbered below:

Post a cohesive scholarly response that addresses the following:

1) Share your project question, as you have developed it thus far (Write the questions exactly as the PIICOT Template provided in the attached file area).

2) Discuss salient elements that inform your project question: theory, current research, nursing practice, and specialty knowledge.

3) Pose any questions or concerns you may have related to developing your project question or literature review. You may also include any other issues related to expanding your expertise within your specialty area.

Integrating Theories

In this course, we have studied various approaches to therapy and explored the utility of each orientation with reference to the client.  For this assignment, take the perspective of a therapist working with clients. This will help you honestly assess your own development and more objectively evaluate the advantages and disadvantages of using an integrative approach to counseling. Answer each of the following questions thoroughly and provide examples where requested to substantiate your points.

  1. Do you think therapists should focus on becoming an expert in one particular counseling approach or should they try to become proficient in more than one approach? Provide an explanation for your answer and illustrate them with examples.
  2. Identify what the three most important characteristics of a successful counselor you consider to be. Describe how these characteristics can be developed or strengthened?
  3. Identify and describe some of the characteristics (from question 2) that are best developed in the counseling process with clients?  Support your points with examples? Identify and describe some areas of personal or professional development that would potentially be unethical for the counselor to develop in the counseling process with clients? Explain your rationale in detail and give examples.
  4. What are some of your own personal motivations and characteristics that may help or hinder you as a counselor (identify at least two)?
  5. What areas of your own personal and professional development need the most development before you will feel prepared to be an effective counselor (identify at least three)?
  6. What are two advantages and two disadvantages of practicing within the framework of one specific theory as opposed to developing a more integrative approach consisting of several therapies? Give a detailed description and rationale for both sides.
  7. What are the advantages and disadvantages of practicing within an integrated framework?
  8. Explain at least two techniques you would use in your treatment and describe how you would integrate these techniques in a therapy session.

Your paper should be double-spaced and in 12 point, Times New Roman font with normal one-inch margins, written in APA style, and free of typographical and grammatical errors. It should include a title page with a running head, an abstract and a reference page. The body of the paper should be no less than 6 and no longer than 8 pages in length. Submit your response to the M5: Assignment 1 Dropbox by Monday, May 23, 2016.(Integrating Theories1)

Traumatic Repercussions

Traumatic Repercussions: Concussion, PTSD, & Neurological Complications

(Traumatic Repercussions)

Traumatic brain injuries (TBI) like concussions can lead to long-term consequences including post-traumatic stress disorder (PTSD) and neurological issues. The impact of TBI on mental health and cognition is substantial, with increased risk of mood disorders and cognitive impairment. Early intervention and support are crucial for recovery (references: Mayfield Clinic, CDC).

Randy Adams is a 38-year-old male patient of Dr. Joseph Reynolds who was admitted yesterday morning for 24-hour observation for mild concussion following a motor vehicle accident. Randy lost consciousness during the accident and was very confused when he arrived in the ER after EMS transport. He is an Iraq war veteran and he seemed to think after the accident that this all happened in Iraq. Dr. Reynolds is concerned that Randy has some residual problems from a couple of explosive incidents that occurred while he was in Iraq. The physician is unsure whether Randy’s current symptoms are from the car accident or from prior injuries so he has referred him for consultations to both a neurologist and to a behavioral health specialist.

Based on the above please discuss the following.

Pathophysiology of concussive injuries and treatment

Neurological assessment tools used in your current practice setting (if not presently working, please describe one used during prior employment or schooling)

Current best practices associated with post-traumatic stress disorder (PTSD)

Nursing interventions you would include in this patient’s plan of care

How can changes in nervous system functioning affect an individual’s psychosocial health status?

What resources are available at the local, state, and federal levels to assist those suffering from PTSD?

While reviewing Randy’s discharge instructions with him, you notice that he is having difficulty holding the pen in his hand and appears to be unable to sign his name. You ask him if he is okay. He looks at you and attempts to smile but only the right side of his face appears to move—the left side is fixed. You fear he may be having a stroke. What would your next steps be?

Human Observation Paper2

Human Observation Paper2

(Human Observation Paper2)

The Human Observation Project should consist of a minimum of five typed pages. Information should be provided for each section of the Observation Project Form. The project is divided into two section:

  • the gathering of baseline information
  • behavior change

Be sure that the project submission adheres to the following formatting requirements:

  • Use double-spacing.
  • Use size 12 font.
  • Set margins to one-inch on all sides.
  • Be sure to include your name/course title on the first page.
  • Write in complete sentences, use good English grammar, and correct spelling.
  • Avoid personal pronouns and statements such as “I believe, I placed the coin on the floor…”, “My research proved that….” – in objective, naturalistic research your opinion is not very important, but your findings are. Your research may suggest that…, support the hypothesis…, or indicate….; but it does not necessarily proveanything.
  • Charts and graphs are part of an “A” paper, but are not part of the basic page count of the project. References to outside sources may also part of an A or B level paper. Information should be provided for each section as outlined below.
  • APA documentation style must be used when citing references in context and bibliography (if any).
  • Key Terms(Human Observation Paper2)

In order to complete the Human Observation Project, you will need to be familiar with the following terms:

    • Statement of the Problem: Explain the problem behavior. Convince the reader it needs to be observed very closely before one could decide how to change the behavior.
    • Theory: This is a prediction. What do you expect to observe. The theory is a general statement. For example, most males or females do not wash their hands after using the restroom. Most people will not pick up after themselves after eating in a public place.
    • Hypothesis: The hypothesis must be written in such a way as to test the theory. A theory is like an umbrella covering behaviors with the presumption that they are related. A good hypothesis rains on the umbrella to see if there are any holes. For example, between the hours of 11:00 and 1:00 on Monday and Wednesday at McDonald’s most patrons (or males, females, adolescents) will not place their napkins, cups, plates, and eating utensils in the trash and return their tray to the rack.
    • Procedure: This is a description of the step-by-step process used during the observation. Where did the observer sit? Was the observer visible to the subject being observed? How was data collected? The description needs to be written in sufficient detail that someone else could attempt to replicate (repeat) the procedure to determine if the same results could be obtained.
    • Results: The results are given in the form of numbers. This is the count. It is often presented in complex statistical terms. A numerical count and percentages will be sufficient for our purposes.
    • Discussion: This is a summary of the results in simpler, more practice language. The numbers are converted to statements of meaning and application.
  • Section 1: Naturalistic Observation(Human Observation Paper2)

The first half of your research will be a naturalistic observation. You will be determining the baseline of behavior, or what the behavior looks like, or the amount of the behavior present under normal circumstances. The observer is unobtrusive, rather like the wallpaper. There is no interference with the behavior.

You are to select a human behavior. Discuss the problem surrounding this behavior. The following is a list of topics which have been used in the past. You may select from the list or develop one of your own. Select a behavior which you encounter each day. The greatest challenge is isolating or narrowing the behavior to a single event which you can define, count, and attempt to change or observe as changed in a different environment.

    • Eye contact
    • Hand washing
    • Door opening for others
    • Money on the ground
    • Cleaning off the table after you eat in a fast food restaurant
    • Response time of clerks when the researcher dresses poorly or nicely
    • Tips – restaurant, beauty salon, etc.
    • Helpful behavior when toilet paper is attached to the researchers shoe in a public place
    • Hand waving when driving down a country road
    • Changing television stations in a public waiting room
    • Products purchased from shelves of different height
    • Color of products purchased
    • Seating behaviors in school cafeteria or restaurant
    • Stop light running
    • Use of cell phones in school areas
    • Use of cell phones while driving
    • Purchasing one item or the “full meal deal” at a fast food restaurant
    • Human responses to walking dogs of different sizes or breeds
    • Human response to “Don’t walk on the grass!” signs
    • Driver behavior while waiting on a stop light (make-up, hair combing using rear view mirror)
    • Assistance reaching items from the top shelf
    • Dropping a dollar while walking through Walmart…will someone return the dollar? (Can be an expensive project.)
    • Returning shopping carts to the proper areas
    • Smiling or waving “thank you” when a car stops to allow shoppers to cross in the parking lot
    • Behavior in the check-out line: smiling, conversation…
    • The behavior of children in the check-out line (pulling things from shelves, yelling, smiling, climbing out of cart….)
    • Behavior of children in a classroom (talking, out of seat, interrupting, turning in homework….)

    DO NOT :

    • Place a baby carrier on top of a car and drive around the mall parking lot to see if someone will attempt to stop you
    • Stop your car by the side of the road to see if someone might stop and assist
    • Attempt a tail gating experiment of any kind
    • Select any behavior which might be harmful, socially offensive, or immoral

Complete the Observation Project Form. Write a theory and hypothesis, explain the procedure you will use to determine if the theory and hypothesis are supported, give the result or the count, and finally, discuss your results or findings.

  • Section 2: Experiment

The second half of the project will be a type of experiment. By introducing a variable, you will attempt to increase or decrease a behavior. For example, one student in Iraq counted the number of men who failed to wash their hands after using the latrine next to the dining facility. This student’s count suggested a problem. During the behavior change section of the project, the student placed honey (the independent variable) on the handles of the doors. As a result, hand washing (the dependent variable) increased.

Your project will require you to walk through the same steps again, but from the point of view of changing or improving the behavior. Begin with your own results. That is the statement of the problem. You have counted and found that, yes, this is an area of human behavior which should be improved. Complete the Observation Project Form. Again, APA documentation style must be used when citing references in context and references (if any). Your textbook may be your only reference.

Discussion Poverty and Discrimination

Assignment 1: Discussion—Poverty and Discrimination

(Discussion Poverty and Discrimination)

In discussing poverty and discrimination, it’s crucial to recognize their interconnectedness. Discrimination exacerbates poverty by limiting opportunities for marginalized groups, perpetuating cycles of disadvantage. Whether based on race, gender, or socioeconomic status, discriminatory practices hinder access to education, employment, and social services, deepening economic disparities. Addressing poverty requires confronting systemic discrimination through policy interventions, education, and advocacy for equity. By fostering inclusivity and dismantling discriminatory barriers, societies can work towards alleviating poverty and promoting equal opportunities for all individuals.

Poverty and discrimination are linked as one often leads to the other and vice versa. The poor are a target for discrimination due to their circumstances, while discrimination due to ethnicity, race, or gender reduces the number of economic opportunities an individual might get. In this module, you explore the segments of the population that are more likely to be poor. You also examine the reasons for the poverty of these segments and its implications for social policy development.

Research the link between poverty and discrimination using your textbook, the Argosy University online library resources, and the Internet.

  • Select two journal articles that address the linkage between poverty and discrimination reflected in the conditions of racial and ethnic minority populations.
    • Be sure to select one article that addresses poverty and discrimination in the United States and one in any other country.
    • Make sure the articles include examples of social policies that have attempted to address these issues.

Respond to the following:

  • How is the linkage between poverty and discrimination reflected in the conditions of racial and ethnic minority populations?
  • Identify two examples of social policies that have attempted to address these issues and discuss their effectiveness.
  • Support your positions with material from your library research.

Write your initial response in a minimum of 400 words. Apply APA standards to citation of sources.

Navigating Sandra’s Family Journey

Navigating Sandra’s Family Journey

Influences of Environment on Behavior and the Family System

(Navigating Sandra’s Family Journey)

Question description

For this Assignment, you will write about Sandra Lombardino’s ecomap that you submitted in Unit 8. In your paper, you will summarize and evaluate the influences of the environment on Sandra’s behavior and family system.

Using the ecomap you created as a resource, discuss the following topics about Sandra and her family:

  1. Provide a one paragraph history about Sandra’s life to this point.
  2. Explain whether Sandra is in Integrity or Despair as defined by Erik Erikson in his Theory of Psychosocial Development. Explore how this transition crisis may be resolved by a HSP conducting a life review with Sandra.
  3. Using Sandra’s ecomap, describe what systems (influences) are causing the most stress and negative influence on Sandra’s behavior and decision making.
  4. Considering Stress Theory, what are some recommendations a HSP could make to Sandra to help her cope with her trauma/stress that may help her meet her goal of a long life expectancy?
  5. If you were working with Sandra and she provided consent to contact her family, do you believe that involving her adult children would make a positive impact to her family system? Explain why or why not.
  6. What role could Sandra’s adult children play in helping Sandra cope with her trauma/stress?
  7. Name one human services agency that serves older adults that may provide assistance to Sandra and her family. Explain how this agency would specifically help Sandra and her family with their current stressors.
  8. Considering all you have learned in HN144, what are some best practice approaches that a human service professional could use to help a multigenerational family experiencing a negative environment?

Your paper should be 8 complete paragraphs, resulting in approximately 2–3 pages of text in addition to the cover page and the final reference page. It also should be free of spelling and grammatical errors. Your Assignment should be double-spaced, using a 12 point Times New Roman font.

Your Assignment should include a cover page and a final reference page where you cite your course text and any other sources you choose to include in the paper. The only required source is your text.

You can explore the Kaplan Library or credible Internet sites for additional sources, but the text is your primary source of support. Your opinions also are valued, but it is important to keep in mind that your main objective in completing this project is to illustrate your knowledge of the course material.

Citing Sources(Navigating Sandra’s Family Journey)

It is important to always attribute credit to the proper sources when relying on the text, articles, websites, or other sources. You must credit your work when you:

  • Summarize concepts and theories that you learned in the text, articles, or internet sources.
  • Use others’ ideas, theories, and concepts in constructing Discussion topic responses or completing Assignments.
  • Use direct quotes in the context of your paper (when quoting directly, you must use quotation marks).

Here is the case study so that you can read it as well and I will upload my ecomap for you that I did for this paper to be done.

Sandra Lombardino is 69 years old. Except for being overweight and having arthritis, she is in fairly good health. She is personable, well-groomed, kind, and articulate. She retired two years ago from her job as an elementary school teacher; she was well liked by students and her fellow teachers in her 33 years of teaching. She raised four children, all of whom have started careers and families of their own. Mrs. Lombardino would like to use her retirement years to travel and do volunteer work. She has worked hard for many years and has looked forward to enjoying her retirement. She is increasingly frustrated because her husband’s demands and offensive behavior are destroying her retirement dreams. Her husband, Benedito, has a number of health-care needs. Benedito used to be a carpenter and at one time was a good athlete. But he has been a heavy drinker for more than 40 years. When drunk, he has been physically and verbally abusive to his wife and to his children. His children left home to escape from him as soon as they were financially able to do so. The children love their mother but despise their father. In many ways, Sandra Lombardino has been a martyr. She took a marriage vow to stay married for better or worse until death. She has fulfilled that vow, despite the urging of her friends and relatives to seek a divorce. Several years ago, Benedito was diagnosed with cirrhosis of the liver and had to stop working. He now receives a monthly disability check. Despite his illness, Benedito has continued to drink heavily and has developed high blood pressure and diabetes. He is grossly overweight and is often incontinent. The drinking and illnesses have caused brain deterioration; he now has difficulty walking, talking, and grooming himself, and he frequently hallucinates. His behavior has resulted in a loss of friends. Benedito has been pressured into attending a number of alcoholism treatment programs, including Alcoholics Anonymous, but he has always returned to drinking. Sandra Lombardino is in a quandary about what she should do. She is angry that she has to spend most of her waking hours caring for someone who is obnoxious and verbally abusive. She resents not being able to travel and to leave home to do volunteer work. Sometimes she wishes her husband would die so that she could get on with her life. At other times, she feels guilty about wishing he would die. She has contemplated getting a divorce, but such a process would mean her husband would get half of the property that she has worked so many years to acquire. She has also considered placing Benedito in a nursing home, but she feels an obligation to care for him herself and realizes that the expenses of a nursing home would deplete her life savings. Mrs. Lombardino feels that the cruelest injustice would be for her to die before her husband dies, so that she would be robbed of her chances to achieve her retirement dreams.

Directions

Within your group, you will work to create a WebQuest. A WebQuest is an inquiry-oriented assignment. Look at sample WebQuests (search online) to see how creative you can be. The format is divided into sections or independent pages. Think of each section as a section on a paper.

For your WebQuest presentation, you will choose a problem that is a public policy issue. You will define the health care problem. You may be working in a certain clinical area and may see a need for more research or alternatives to treatment options. The problem may be a professional problem that you as an advance practice nurse are encountering in terms of autonomy or reimbursement for your services. It could be a workplace safety issue or a management issue. It could be a general societal problem that needs to be brought to the attention of the legislators.

Your WebQuest Presentation:

  • Most of the information for your WebQuest will come from the Web. However, you are expected to use the Kaplan Library to supplement the information that you find on the Web.
  • You must also use a minimum of three graphics or images to illustrate your points. The graphics can go on any of the pages in the WebQuest and you can use more if you like. The graphics do not substitute for the text that is to explain each section.
  • You are expected to use analysis in each of the sections and provide details and explanations in each area. The Web resources are to validate what your position or argument is.
  • You will use this WebQuest PowerPoint template to build your WebQuest. Before you begin your work however, watch this video to learn how to edit the template to create your unique WebQuest.

Movenote Presentation

In one section of your WebQuest, you will include a link to a Movenote presentation that you create. Movenote is an online tool that allows you to combine slides (e.g., PowerPoint, Google Slides, or Prezi slides) and video, displayed as an inset. The video will be of you, on camera, as you narrate. In this way, you can combine the two, presenting your ideas in video as you display your slides. Be sure to review How to Use Movenote for Your Assignment Presentationbefore you begin.

In your Movenote presentation, you will give a 5-minute talk with slides, as if you were speaking at a press conference delivering your message to a politician or to the community as a whole about what you have identified as the problem. Your presentation should explain why your identified problem needs to be addressed.

The following should be incorporated in your WebQuest: The two that I bolded are the ones that I need the most information placed on the others I do not thank you.

  • The health care problem/public policy issue
  • A link to your Movenote presentation
  • A transcript of the Movenote presentation
  • What the government’s response to this problem has been previously (i.e., what has been done in terms of laws, rules, regulations, or programs; if the issue has never been addressed, you will discuss what needs to be done)
  • Your assessment of the legislative and policy-making strategies that influence health care services
  • Your proposed bill, either on the state or federal level, advancing your proposed change
  • Why these changes are important and a discussion of what opportunities nurses have to become involved in the design of this program
  • Your advice on how the program can be implemented
  • A summary of the Project
  • The credits page

Assignment 1 Section A(Evidence Based Practice Proposal )

Evidence-Based Practice Proposal – Section A: Organizational Culture and Readiness Assessment

Details:(Evidence Based Practice Proposal )

Before making a case for an evidence-based project, it is essential to understand the culture of the organization in order to begin assessing its readiness for EBP implementation.

  • Complete the “Organizational Culture and Readiness for System-Wide Integration of Evidence-Based Practice Survey,” located in the textbook appendix.
  • Develop an analysis of 250 words from the results, addressing your organization’s readiness level, possible project barriers and facilitators, as well as how to integrate clinical inquiry.
  • Make sure to include the rationale for the survey categories scores that were significantly high and low, incorporating details and/or examples. Also explain how to integrate clinical inquiry into the organization, providing strategies that strengthen the organizations weaker areas.
  • Submit a summary of your results. The actual survey results do not need to be included.

ASSIGNMENT 2Evidence Based Practice Proposal

Evidence-Based Practice Proposal – Section B: Problem Description

Details:

Write a paper of 500-750 words (not including the title page and reference page) on your proposed problem description for your EBP project. The paper should address the following:

  • Describe the background of the problem. Tell the story of the issue and why it deserves attention.
  • Identify the stakeholders/change agents. Who, or what organizations, are concerned, may benefit from, or are affected by this proposal. List the interested parties, patients, students, agencies, Joint Commission, etc.
  • Use the feedback from the Topic 2 main forum post and refine your PICOT question. Make sure that the question fits with your graduate degree specialization.
  • State the purpose and project objectives in specific, realistic, and measurable terms. The objective should address what is to be gained. This is a restatement of the question, providing focus. Measurements need to be taken before and after the evidence-based practice is introduced to identify the expected changes.
  • 5) Provide supportive rationale that the problem or issue is an important one for nursing to resolve using relevant professional literature sources.
  • Develop an initial reference list to assure that there is adequate literature to support your evidence-based practice project. Follow the “Steps to an Efficient Search to Answer a Clinical Question” box in chapter 3 of the textbook. Use “NUR-699 Search Method Example” to assist you.
  • 7) The majority of references should be research articles. However, national sources such as Centers for Disease Control and Prevention (CDC), National Center for Health Statistics (NCHS), Department of Health and Human Resources (HHS), or the Agency for Healthcare Research and Quality (AHRQ) and others may be used when you are gathering statistics to provide the rationale for the problem.
  • Once you get into the literature, you may find there is very little research to support your topic and you will have to start all over again. Remember, in order for this to be an evidence-based project, you must have enough evidence to introduce this as a practice change. If you find that you do not have enough supporting evidence to change a practice, then further research would need to be conducted.
