After reading the case examples in the Myer and Conte (2006) article, you have a better understanding of how to use one type of assessment tool. A Microsoft Word copy of the Triage Assessment Form (TAF) is included in the assignment Resources. The most current version of this form is also shown in your James and Gilliland (2017) text, pages 60–64. Use the form to analyze one of the cases, either Ariadne or Jordan, described below. You can save the form as you have completed it as a MS Word document or as a PDF document, and attach the form to your written paper as an appendix.

Rate the client in each of the three domains (Affective, Behavioral, and Cognitive) using the Severity Scale included with each domain on the Triage Assessment Form (TAF) and total the scores. Describe, in detail, the rationale for your ratings, including your judgment about how intense and directive the treatment should be based upon the total score. In your discussion of the rationale, summarize diagnostic skills and techniques that can be used to screen for addiction, aggression, and danger to self and others, as you note these risks in your client. Similarly, a possible co-occurring mental disorder (such as substance abuse) may become apparent during a crisis, disaster, or other trauma-causing event that ties in with your assessment during the client’s crisis. Note this in your rationale to address the impact of crisis and trauma on individuals with mental health diagnoses.

Project Objectives (Triage Assessment analysis)

  • Complete the Triage Assessment Form appropriately for the selected case, including all three domains, with clinical descriptions to guide the course of treatment by evaluating the domain ratings with a logical and articulate rationale of key elements of the crisis, disaster, or trauma-causing events, including the nature of the crisis and associated risks, and client and counselor safety.
  • Summarize diagnostic skills and techniques that can be used to screen for addiction, aggression, and danger to self and others, as you note these risks in your client.
  • Note a co-occurring mental disorder (such as substance abuse or depression), which may become apparent during a crisis, disaster, or other trauma-causing event that ties in with your assessment during the client’s crisis.
  • Differentiate characteristics of crisis states versus developmentally appropriate reactions to life obstacles and crisis assessment and intervention strategies for diverse populations.
  • Exhibit proficiency in effective, credible academic writing, and critical thinking skills.

Case of Ariadne:

Ariadne, a 17-year-old Hispanic female, ran away from home. The police returned her to her home, but within a week Ariadne had attempted suicide by taking her father’s prescription medication for high blood pressure. Ariadne had been showing signs of depression and was seen for mental health counseling a year previously for eight sessions. After receiving counseling, Ariadne stated that she felt unuseful at home and unwelcome at school. Feelings of worthlessness and anger arose periodically when her parents tried to engage her about school events. Ariadne had several close friends and one young man she called her “beau,” though she claimed there was no serious intimacy between them. She refused to return to counseling sessions, saying that the time was better spent talking with her friends. She complained that her parents were too strict with curfew times and asked too many questions. In the past week, Ariadne was discovered to skip school two days and refused to tell her parents where she had been. Ariadne’s mother found a bottle of pills and a bottle of vodka in her room.

Using the Triage Assessment Form

Include the title of your paper centered at the top of the page, not bolded; it is not considered a heading. *This first section is your paper’s introduction.

Triage Assessment of the Client

Complete the Triage Assessment Form for the selected case, including all three domains and the total score. In this section of the paper, summarize the results and provide a logical and articulate rationale for each of the domain ratings with specific descriptions of each, by relating the specifics of the case to the ratings you determine. There is detail about using the TAF in Chapter 3 of your text, as well as the assigned Myer and Conte article. Use appropriate terminology, such as the psychobiological assessment found in Chapter 3 of your text, and language found in the TAF Severity Scales, to guide the course of treatment based upon your total score.

 

Diagnostic Skills and Techniques

Elaborate on diagnostic skills and techniques that can be used to screen for addiction, aggression, and danger to self and others, as well as co-occurring mental disorders during a crisis, such as the Hybrid Model and the ABC’s of Assessing Crisis Intervention found in Chapter 3 of your text. Discuss what counseling skills you use in a triage assessment of this client.

Developmental and Cultural Considerations in Crisis Assessment and Intervention

In this section of the paper, describe how you would differentiate between the characteristics of crisis states versus developmentally appropriate reactions to life obstacles. Describe crisis assessment and interventions considerations and strategies when working with diverse populations. Consider any cultural, diversity, or even gender issues that may be involved in assessment or intervention with your chosen scenario. Give examples of what you would include in your assessment and intervention.

Based on your review of the most recent, relevant research findings on your chosen topic, submit a final version of your literature review paper, integrating feedback received from your instructor.   • Begin the review by defining the objective of the paper. Introduce the reader to your focal question. What is this question intended to address? You may state your “question” in the form of a problem if you like. Describe the topic for your literature review and why you chose this topic. Explain why you think it is important. It is also useful to tell the reader how the review is organized in your introduction section, before you the transition into the body of the review.
  • Organize your literature review paper by themes/theories/concepts, rather than article by article. If there is one major theme you want to highlight, state the theme. If there are three major themes or streams of thought on the topic, briefly name them—and then organize the balance of your literature review around those three streams. Ageist attitudes can cause people to think elders are not sexually active, resulting in a failure to recognize their symptoms of STD or give proper patient education.

 

2-A clinician’s knowledge of an elderly patient’s everyday concerns, social circumstances, mental function, emotional state, and sense of well-being helps orient and guide the interview. Asking patients to describe a typical day elicits information about their quality of life and mental and physical function (Brink, 2014). This approach is especially useful during the first meeting. Patients should be given time to speak about things of personal importance. Clinicians should also ask whether patients have specific concerns, such as fear of falling. The resulting rapport can help the clinician communicate better with patients and their family members (Brink, 2014). A mental status examination may be necessary early in the interview to determine the patient’s reliability; this examination should be conducted tactfully so that the patient does not become embarrassed, offended, or defensive (Brink, 2014). Routine screening for physical and psychologic disorders should be done annually, beginning at age 70.

 

3-Disorders that are common among the elderly are frequently missed, or the diagnosis is delayed. Clinicians should use the history, physical examination, and simple laboratory tests to actively screen elderly patients for disorders that occur only or commonly in the elderly; when diagnosed early, these disorders can often be more easily treated (Brink, 2014). Early diagnosis frequently depends on the clinician’s familiarity with the patient’s behavior and history, including mental status. Commonly, the first signs of a physical disorder are mental or emotional. If clinicians are unaware of this possibility and attribute these signs to dementia, diagnosis and treatment can be delayed (Brink, 2014).

 

4-I found myself doing the same thing working on the oncology unit. I would float to the floor as an agency nurse and would automatically get depressed in the report. Hearing all of those sad stories of the young patients and their prognosis. Just to see these young people dying with babies at home, it did something to me. I told my staffing agency to please remove it from my profile. I didn’t care if I was to go hungry, I couldn’t do it. I still cry when I hear a grieving child crying at their mother bedside. It does something to me.

5-my family had a similar situation to yours. I think it happens to many people. No matter how much they would love to just drop everything and fulfill the wishes of their dying loved one, it’s just not possible. There are still the children to take care of and job obligations. Knowing this helps us to encourage our patient’s family that there are many other ways they can continue to show their love to a dying relative.

 

6-Thank you for sharing your personal experience with your Grandpa. I can imagine how difficult it must have been for you and your family. I can understand how your mom would have been far too stressed with her grieving process of her partner and then to prepare to go through it all again too soon would have been hard.  I think as families, we do the best we can and hope for the best. As nurses, we have to understand that some families just can’t drop their lives to be at the bedside. It’s not that they don’t care or love the person, it’s just life.

The literature review process can be an inexact science and tedious process because of the extensive scope of public health material, studies, and research. To add to this challenge, there are inconsistencies in the search process which demands a systematic approach to reviewing available literature in order to apply evidence-based decision making. Nonetheless, the literature review remains a key process in EBPH.

Going back to Week 3 Project in which you identified five peer-reviewed articles, use the other two articles that you did not use for your assignment. Now, do the following:

  • Utilize these two articles to “Analyze, Summarize, and Apply the Literature Review” process.
  • Summarize each article ensuring that you focus on the relevance of the article to your public health topic. Each summary must clearly demonstrate the connection or importance of the article being reviewed. For example, one article may be providing background information to your public health problem while another article may be presenting policy approach to your public health problem while yet another article may be reviewing a successful intervention or approach to address your public health problem.

Each student will prepare a portfolio as a requirement for graduation. The portfolio is used to assess student achievement of Eight End-of-Program Outcomes. End-of-Program Outcome #7 challenges us to incorporate professional values in formulation of ethical comportment and personal accountability for nursing practice. In other words, what are the essential lessons you have learned about being a good nurse?

Webster’s Dictionary Definition or Comportment (n.): Manner of acting or controlling yourself, dignified manner of conduct.

For this assignment, select one of the following topics and write about your formative learning experience with the everyday ethical comportment of nurses.

1. Meeting the patient as a person: share a story of meeting a patient as a person and not as a diagnosis and how you connected with the patients’ vulnerabilities.

2. Preserving the dignity and personhood of patients: share a story of witnessing the assaults on the dignity of a patient inflicted by health care routines, treatments, and objectification. Describe how you have preserved that dignity and personhood.

3. How to respond to substandard practice: share a story of an encounter with substandard practice encountered in practice and your response or desired response given the challenge to behave professionally.

4. Patient advocacy: share a story of how you have provided patient advocacy. A story of giving voice to a patient, empowering a patient, clarifying confusion about treatment, making sure there are no dangerous drug interactions, allergies, or contraindications, or any other ways of advocacy.

5. Engaged in doing “good” in nursing practice: share a story of how you planned to improve your nursing practice for the second day you cared for the same resident (based on the care provided or not provided on the first day).

6. Being present with patients and families in their suffering: share a story of how you worked to alleviate suffering. Share how a patient’s pain, fears, and concerns moved you to be attuned to the patient and comfort them without being intrusive.

1- How does the epidemiologic triad apply to health issues we see in the hospital? What are some common diagnoses in the acute care setting? What about in the community setting? How does the epidemiologic triad differ when a nurse is providing care to someone in the community or in his home? Does the nurse have more or less control in either arena?

2- Discuss the data presented regarding obesity in America in the Ethical Connection feature on page 118.  Please review the data and discuss your feelings about the role of community health nurses in community nutrition.

3- What are some of the causes for increases in healthcare costs in recent years, as outlined in the chapter?  Brainstorm some examples that you have actually witnessed in the clinical setting. Can any of these factors be modified? What could nurses do to help cut down on costs related to the different factors?

4- Why are the causes of morbidity and mortality in other countries different than in the United States? In what ways are they the same? Why do those differences exist? What types of services & interventions are needed in less developed countries to help with their health issues?
