Organizational Culture and Values

Introduction

Organizations rely on managers and leaders to fulfill their mission today and their vision of the future. The focus for this week will be on the roles of nurse managers and leaders in health care organizations, theories that underlie the practice of management and leadership, as well as the use of power in an organization.

Role of the Manager

The role of the manager is to ensure that the mission of the organization, which focuses on providing excellent care for clients, is fulfilled through the effective and efficient coordination of resources. Managers are responsible and accountable for ensuring that competent staff are provided with the tools and processes required to accomplish the work. To perform this role, managers are given the authority to make decisions that directly influence these factors within their scope of responsibility.

The functions of the management role include planning, organizing, staffing, directing, and controlling (Marquis and Huston, 2009). Within each of these functions, decisions must be made to optimize the care provided while maintaining fiscal responsibility. Planning is required to determine the best ways to fulfill the organization’s mission. As in client care, this step includes assessing the current situation and identifying actual or potential issues. Organizing provides the framework within which care is provided. Staffing refers to determining both the overall number and skill mix (e.g., numbers of licensed and unlicensed personnel required to care for a specific client population) needed as well as ensuring adequate shift-to-shift staffing. Directing includes assuring the work is being accomplished, whereas controlling encompasses both quality management and adherence to the budget. In this course, elements of each of these functions will be explored.(Leadership and management discussion 4)

Role of the Leader

The role of the manager is often viewed as one that works to maintain the status quo and ensure smooth day-to-day operations which are critical to the organization. A leader is viewed as one who encourages growth in the organization. The word itself implies movement and there is no need for a leader to simply get people to where they already are.

Leadership remains a vague concept, but ideas about what makes a great leader abound. Warren Bennis offers this definition “Leadership is a function of knowing yourself, having a vision that is well communicated, building trust among colleagues, and taking effective action to realize your own leadership potential” (The Teal Trust, n.d.). Throughout the definitions and discussions of leadership, two major themes emerge: 1) leaders are responsible for promoting growth, and 2) leaders work by influencing and empowering others.

Theories regarding leadership styles and their application also abound. Early in the discussion of leadership, three basic styles were defined. These classic types include authoritarian, democratic, and laissez-faire. Further exploration of these styles led to a theory of situational leadership, where the leader alters their approach based on the issues and people involved. Knowing the advantages and disadvantages of each approach and matching these to the situation should achieve better outcomes.(Leadership and management discussion 4)

Current theories of leadership focus on recognition and empowerment of individuals. The challenge lies in developing a culture that fosters these collegial relationships and in preparing individuals for leadership. Not everyone in a leader’s circle of influence will be interested in becoming a leader. Some find it difficult, due to their culture or experiences, to develop a different relationship with persons viewed as authority figures (Marquis and Huston, 2009). Despite the challenges, this approach to leadership of professional staff shows promise in creating a work environment that enhances nurse satisfaction (Kerfoot, 2004).(Leadership and management discussion 4)

Power and Politics

Power is defined as “the ability or capacity to act or perform effectively” (The American Heritage Dictionary, 1985, p. 971). Power is also linked to the ability to influence or control. Power is strongly linked to the roles of manager and leader since both, by definition, must be able to influence or control others in order to accomplish the mission of the organization.

People within the organization gain power in different ways. Managers are given legitimate power, otherwise known as authority, to provide rewards and consequences for staff behavior. It is well known that being given authority does not always lead to the power to manage others, as seen when staff choose not to follow policies. Bridging the gap between authority and power often requires the skill of a leader to influence people to work together and be willing to follow policies for the good of the organization or speak up when they believe the policy no longer benefits the organization. Leaders may or may not have legitimate power or authority. Leaders in this position often influence others through expert and referent power. This type of leader may be a member of the staff who is recognized as a clinical expert or who is aligned with persons in authority.(Leadership and management discussion 4)

Marquis and Huston (2009) define politics as the effective use of power. Politics recognizes that all people are interdependent; no one can accomplish the work of the organization in isolation. Learning the political climate of an organization is a key to success as a manager or leader. Developing relationships with others in the organization is a critical component. This is best done in face-to-face encounters rather than by telephone or e-mail. Possessing information is also a form of power, knowing when and with who to share that information is also important. A new manager or leader will often benefit from finding a mentor who understands the political climate of the organization.

Integrating the Roles of Manager and Leader

Not all leaders are managers and not all managers are leaders. The theories of transactional and transformational leadership highlight this quite well. A transactional leader is interested in maintaining the status quo. A transformational leader is interested in promoting growth, both for themselves and for others. Although a manager is given legitimate power simply by the authority delegated to the position, this is often not sufficient for truly carrying out the mission and vision of the organization. Conversely, leadership alone, without attention to day-to-day organization, is likely to lead to chaos. Finding a balance of the roles that suit the organization and the people being managed or led is the challenge.(Leadership and management discussion 4)

Conclusion

Although managers and leaders have distinct roles within an organization, the most effective people will blend the functions and roles in their work. Both managers and leaders need to develop a power base and use that power wisely within the organization to further the mission and goals. When management is effective and efficient, and leadership is characterized by vision, communication, and empowerment, then both the organization and the clients served will benefit.

One of the reasons for this new emergence of the topic of mental health in the mainstream media is because of worldwide traumatic events. Many tragic events covered in the media are centered on one or two individuals who have committed acts of violence. Unfortunately, most of these individuals have had a history of mental illness. This aspect of the tragedy then becomes the focus of analysis within the media. From this point of reference, reporters and journalists educate the public on mental health issues. However, these tragedies could be an opportunity to educate the public about the facts related to mental illness. Unfortunately, the media discussions about mental illness often result in attaching a stigma to mental illness and to the population suffering from mental illness.

 

As a clinical social worker, you need to be aware of the impact of the entry of psychopathology and diagnosis in mainstream public discourses. You also need to think about how the media could help eliminate or mitigate the stigma attached to mental illness in the mind of the general population.

Historical Divides and Ethical Obligations Within Social Work

The National Association of Social Workers (NASW) specifies the ethics and values of the profession in their Code of Ethics. Section 6.04 of the Code of Ethics (1999) states:

Social workers should engage in social and political action that seeks to ensure that all people have equal access to the resources, employment, services, and opportunities they require to meet their basic human needs and to develop fully. With the requirement of social and political action among social workers, there is little political activity among clinical social workers. Why? What barriers exist that prevent social workers from fulfilling this ethical obligation?

 

What does it mean to be a macro social worker? A micro social worker? Do these differentiations have any real meaning? If not, why is this language used when referring to the social work profession?

Case Study

C.S. was a 49 year old female with severe peripheral arterial disease (PAD) who had underwent numerous operations to reestablish circulation in her legs. I was taking care of her every few months or so when her stent would clot off or she would need another bypass in her legs. She took her Plavix and aspirin daily and could not understand why she was continuing to have problems.

Pharmacokinetics, Pharmacodynamics, and Pharmacogenetics

Clopidogrel (Plavix) is a prodrug, meaning is must be converted by the liver into its active form using the cytochrome P-450 enzyme system (Arcangelo, Peterson, Wilbur & Reinhold, 2017).  The most active enzyme in the metabolism of this drug is the CYP2C19 enzyme (Polasek, 2011). After conversion, it works to decrease platelet activation and subsequent aggregation and is commonly used in stroke, cardiac, and PAD patients. The binding of the active metabolite is not reversible and continues for the lifespan of the platelet (7-10 days), therefore it is important to stop the drug 7-10 days before procedures like surgery (Drugs.com, 2012).

Pharmacogenetics is the study of how an individual’s genotype effects how they respond to the drug (Arcangelo, Peterson, Wilbur & Reinhold, 2017).  As a drug that relies on the body to produce the active metabolite, Clopridogrel depends upon a “healthy” or un-mutated CYP2C19 enzyme. Some populations carry an allele that decreases or even stops the activity of the CYP2C19 enzyme and subsequently makes drugs like Clopidogrel much less effective (Han et al., 2015).  This can account for up to a 70% variability in the response to Clopidogrel and may be the reason C. S. is experiencing repeated stent thrombosis (Polasek, 2011). The availability of genetic testing can help practitioners individualize drug regimens to optimize care for patients like C.S. (Scott, 2012).

Plan of Care(Need Response To Below Posting)

For C.S., genetic testing might benefit her if it is available and affordable on her insurance plan. I would see if her family history includes PAD and if her family had issues with stent thrombosis as well. If genetic testing is not an option, changing her medication regime to something that does not rely on the CYP2C19 enzyme, such as ticagrelor (Brilinta), would be recommended. With any platelet inhibitor, she must be educated on the risk of bleeding and bleeding precautions for home. To reduce the risk of drug-herb interactions on bleeding time, I would screen her home med list for over the counter supplements such as Omega-3, garlic, ginger, etc. that may interact with the medication and increase bleeding risk (Arcangelo, Peterson, Wilbur & Reinhold, 2017).

Your care plan should be based on current best practices and supported with citations from current literature, such as systematic reviews, published practice guidelines, standards of care from specialty organizations, and other research based resources that are no later than 5 years old. In addition, you will provide a detailed scientific rationale that justifies the inclusion of this evidence in your plan. Your paper should adhere to APA format for title page, headings, citations, and references. The paper should be no more than 10 pages typed excluding title page and references. Some critical point to include in the case analyze are:

1. The patient just had a heart attack and he was treated for it emergently in the ER/hospital.  Many of you need to recall that when a patient is sent to his primary care giver after such an event, a thorough review of the system that is in trouble is completed.

So you should mostly be focusing on the cardiac issues.  That is why the ER doc sent him your way – for long term management of the heart.  The ER doc did not review all of his medications and place him on everything that he needs to be on – that is for you to do and determine.

So that is my first focus – how does his heart look?  well, look at the blood pressure – it is horrible.  So his tenormin is no good and either needs to be changed or increased or something added.

2. Recent history – he had stents placed.  He is only on baby aspirin -That is no longer the standard of care with stent placement. This guy needs to be on clopidogrel or some such blood thinner to avoid complications with his stents forming clots.  That is a lifetime deal – so he needs education about the medication changes, why you are changing them, what they are and how to take them.  He will need some good followup care instructions, and he will need to be strongly advised against stopping the medications for any reason. I would also explore his ability to get the medications and have a steady supply because he might have issues and maybe that is why he is reluctant to maintain them.

4. Then discuss some of his other issues in the overall picture of heart problems – possibly his weight – which at 220 pounds and 5’10” he is not too horribly overweight although his BMI ranks him at obese – just barely.  He could stand to lose 20-30 lbs. largely because he is android obese – tends to hold the excess weight around his midline.

5.  Diabetes needs to be regularly monitored and I would review this with him.  His hgA1C is 7.5 which likely means he averages blood sugar counts of between 150-175.  He may need to revert to insulin to control his sugars if he is not able to control diet enough to allow his glucophage to work. The glucophage will not do all the work – he has to help.

Theory implementation highlights the artistry of counseling, meaning that there are factors you may encounter that may necessitate the integration of other theories into your main theoretical orientation. For instance, client factors such as degree of organization or disorganization as a system may require modifying your selected theory to best assist a couple or family. Personal factors, such as specific experiences within your own past relationships, also may sensitize or desensitize you to the relational issues presented by clients. The key then is recognizing these challenges to integrating theory and knowing how to mitigate them in order to provide the best and most effective service to clients possible while still retaining professional integrity concerning your theoretical orientation.

You are the manager of a new primary care clinic located about twenty five (25) miles outside of a small city (population of 50,000). However, there are still many residents who struggle to make ends meet with older farms that have belonged to families for generations.     The central city includes two (2) large acute care facilities, and one (1) tertiary care facility that is known for its excellent pulmonary care. Both acute care hospitals provide the usual services such as labor and delivery, outpatient surgery, chronic diseases care, etc. and have fully equipped ancillary departments, such as lab and radiology. Up until this point, the residents have used the facilities’ emergency departments for routine illnesses and conditions when their private physicians were not readily available.  Write a five to seven (5-7) page paper in which you:

1.Analyze some of the key social, political, and economic factors that have led to the proliferation of urgent care facilities and primary care practices over the last 20-30 years.

2.Create a comprehensive mission statement for the clinic, and discuss how it will facilitate the provision of quality services.

3.Analyze and discuss one (1) or more directions the clinic might take to grow its business. Determine what factors you would consider when deciding what services to provide in-house and which ones to affiliate with other institutions.
