Task(Certified Nurse Educator)

Kelly, a new faculty member who recently completed a Master of Science in Nursing (MSN) with a focus on Nursing Education, has been an instructor on a team of nursing instructors for six months. Details about Kelly’s work environment include:

  • The faculty team working with Kelly is comprised of four instructors with experience ranging from two to 15 years.
  • One faculty member Edith, the longest residing member of the nursing faculty has a PhD, one faculty member has a DNP and has only been on the team 6 months longer than Kelly, and the remaining two faculty members have a MSN: Manny acting as the simulation and lab coordinator for the past four years and Andre, a staff nurse at the local hospital on joint appointment with the School of Nursing.
  • The program is a prelicensure Bachelor of Science in Nursing (BSN) program with a traditional entry for four years and a two year accelerated option.

The BSN program recently received a grant supplying funds for the nurse educators on staff to take the certified nurse educator (CNE) exam. No faculty member is currently a CNE. Recently, at a weekly faculty meeting Kelly presented a faculty enrichment program titled: “Prepare for the CNE Exam”. The presentation was a success with the faculty who signed up to take the CNE exam. The Dean was excited and asked Kelly to continue the weekly series regarding CNE preparation. 1-I know here in Oregon a free breakfast is given to all children no matter the parents income. If the child falls under the poverty level they can get free or reduced lunch. The school for years ago, installed a backpack program for kids in a very low income family. The packs were filled with food to last the kids through the weekend or on long weekends or breaks. Oregon also has the free lunch program in the summer for all children under the age of 18 Monday through Friday. We do have poverty and homeless, but we do our best to take care of our community members.

 

2-What I have found in the past to be very frustrating is the fact that I have seen a family of 6 get food stamps with a monthyly balance of just under $800. I don’t even spend close to that amount per month for my family of 4. That is plenty of money to buy very, very healthy! I know that the amount families get depends on family size and income. I have also been in line with people infront of me buying prime rib, lobster, and other expensive items and then pay with food stamps. Those things are healthy, but how is that using the money wisely to last the rest of the month. There might be a problem with parents also just buying convience food just for that fact, they don’t have to send time to prep and then cook the food. I know, there are families out there that use the money responsibly and try to be as healthy as possible. 3-We are hearing more and more about childhood obesity and how it is becoming more prevalent in our society today. School age children are at a critical point health wise in their growth cycle when it comes to the ability to grow excess fat (Lee, 2012).  According to Daniel L. Millimet, Rusty Tchernis, and Muna Husain,  childhood obesity in school aged children has risen from 4 percent to 18.8 percent in children ages six to eleven (2018, p. 641). It has been found that children in lower income families and minority neighborhoods are more likely to be obese due to less access to larger grocery stores and more fast food restaurants or convenience stores being close by (Lee, 2012). Larger grocery stores are more likely to have higher quality foods and produce and more healthy choices than smaller grocery stores or convenience stores (Lee, 2012). Convenience stores are more likely to carry unhealthy food choices as they more often than not stock foods that will last longer on the shelf and have a lower chance of perishing (Lee, 2012). Smaller grocery stores may carry produce and fresh food but they will likely carry less choices as compared to larger supermarkets (Lee, 2012). The prices in smaller grocery stores or convenience stores will likely be higher and the quality of the food may be poor (Lee, 2012).  The majority of Americans spend a large portion of their grocery budget on pre-made foods or take out and these foods have a higher calorie content than foods prepared at home. In addition, children who eat out often, consume 150 calories more per day than children who do not (Lee, 2012). One study showed that where there were more convenience stores within a specific zip code, the average BMI of the teenagers in that area was higher than the BMI of children who had more supermarkets within their zip code (Lee, 2012). Families without cars or access to transportation are more dependent on the local food retailers in their immediate area but when the local retailers are more likely to offer unhealthy choices, this becomes a problem (Lee, 2012). It has also been found that lower income families are more likely to drive to a discount grocery outlet or store instead of their local supermarket because the prices fit their budget better but they in turn get lower quality foods (Lee, 2012). They are more likely to choose generic foods and will buy non-perishable foods in bulk (Lee, 2012). As you can see, there are health disparities associated with low income families and minorities when it comes to nutritional choices and weight. Another study done shows the correlation between childhood obesity and the physical home environment in low income families. Children who have more screen time and who do not get enough sleep are more likely to become obese (Appelhans et.al., 2014) The study stresses the importance of implementing a bed time routine in children, reducing chaos in the home and removing televisions from children’s bedrooms to combat the increased rates of obesity in lower income families (Appelhans, 2014) The study showed a direct correlation with obesity, lack of sleep and increased screen time (Appelhans, 2014)

If not if she would like any translator? If she is not being escorted by one. This will be followed her explanation of her condition in full detail. It is important not to judge the patient due to their difference in culture. The medical procedure that will be given will be explained in full detail to the patient and asked if she will be comfortable with the process. Abuse screen is a tool that nurses can use to evaluate if patient is victim of abuse the screening question will be performed to assess how recent how serious family abuse was.

In the case of abuse, she will be encouraged to express her problems. For a proper healthcare delivery in any setup, cultural competency of a nurse is an important factor (Jarvis, 2016). The nurse should clearly understand the cultural beliefs and the type of family setting that the patient was raised in for him to provide the best care to Ms. Li.  Most Asian American families have strong cultural beliefs and this can substantially influence the decision-making process by the patient. Other Asian American families have washed away their strong beliefs and hence their approach to healthcare might be more Americanized or westernized. This background information is important for a nurse since it helps in a proper interview to make an assessment.(Academic Reference Need)

Asian communities are family oriented and like coexisting peacefully as a group as opposed to emphasizing individuality. This coexistence is a protection against the hardships of life and a major source of identity (Jarvis, 2016). Inner strength and stamina and respect to the family are important values for the Asian children. These important values are enforced by their parents, who expect the child to strictly follow the rules and only speak when it is necessary. Given this background, therefore, Asians maybe not willing to acknowledge strong pain, emotion, and grief because of their cultural values and family.

Of the 238 participants, 204 responded to the question, “What is your definition of a professional counselor?” As expected, given the broad nature of the question, participants defined counselors in a variety of ways. In our analysis of the responses, three categories emerged: (a) counseling tasks and services provided, (b) counselor training and credentials, and (c) wellness and developmental focus. A description of each category is provided next, with the most commonly cited categories noted first. One interesting finding in the open-ended responses was that the majority (97%, ? = 197) of participants described professional counselors in general terms, with only seven people describing counselors as defined by their specialization. Three participants defined professional counselors as school counselors, and four defined them as mental health counselors.

Counseling tasks and services provided. This category emerged from the most frequently cited responses, in which participants (78%, n = 1 59) defined professional counselors by what they do on a daily basis and by the services they provide clients. One respondent wrote, “[A professional counselor] offers a variety of counseling and counseling-related services (e.g., psychoeducation, consultation, advising, coaching, advocacy) … to assist clients with personal and professional growth.” Another counselor summed up the responses of a number of participants by writing,

A professional counselor is a clinician who can demonstrate proficient knowledge and skill of those mental health, psychological, and human development issues that commonly occur in counseling through the competent display of cognitive, affective, behavioral, or systematic intervention strategies that address wellness, personal growth, [and/or] career development, as well as pathology.

The focus in this category was on providing a variety of counseling services to clients to help facilitate change in their lives. As one participant noted, “A professional counselor works with individuals, groups, families, and couples to help them identify and address any issues that may be interfering with or impairing their functioning.”

Counselor training and credentials. In this category, participants (49%, n = 1 00) defined professional counselors by their training and credentials, citing the importance of master’s-level training in an accredited program, earning the NCC credential and/or state licensure as well as other relevant state credentials, maintaining involvement in professional organizations, and participating in continuing education. As one respondent noted, “A professional counselor is an individual who has completed a graduate degree in counseling and has obtained state and/or national certification/licensure.” Within this category, a subset of participants specifically noted part of being a counseling professional was following a counseling code of ethics. For example, one respondent wrote, “A counselor [is someone] who has achieved a set standard of testing, clinical hours, and supervision. A professional adheres to ethical standards and continues with ongoing education.”

Wellness and developmental focus. The next category to emerge (12%, n = 25) defined professional counselors as using a wellness and developmental focus in their work with clients as opposed to using a medical or pathology-focused approach. As one respondent wrote, a counselor is “a helping professional with a wellness orientation whose connection with another allows that person to achieve greater selfunderstanding and adjustment to bio-psycho-social developmental milestones.” Another noted, “[Counselors] complete interventions to promote wellness and restoration of optimal levels of functioning.”
