Gloria has recently graduated college and started a new job. Her coworkers seem very nice, and Gloria has gotten to know Lakeisha and Reggie the best.

Reggie is a friendly older man who often talks about his plans for retirement in five years. He likes to make people laugh but often jokes about his struggles to learn new programs and technology.

Gloria and Lakeisha have been assigned to work on a project together. Lakeisha is very organized and has prepared a list of to-do items along with due dates. She even color-coded the list to indicate which partner will do each item. Gloria, who is more laid-back, feels a bit uncomfortable with this micromanaging from Lakeisha, who has only been at the job a few months longer than she has. Gloria wonders if Lakeisha thinks she is either lazy or stupid, and she does not look forward to working on this project. In the past, Gloria attempted to communicate her feelings with a co-worker; however, her coworker told Gloria that she needed to put her feelings aside and just do the work assigned to her.  As Gloria ponders having a conversation with Lakeisha, she starts to feel overwhelmed and thinks she might have to ask to be removed from this project.

Still, she and Lakeisha have connected because they are both single mothers of teenagers. Gloria confides in Lakeisha about her 16-year-old son who has started making poor choices and is currently grounded for sneaking out of the house and getting drunk at a party the night before a big exam.

The company recently informed the team that there will be a compliance test on new safety policies, which employees will need to pass in order to keep their jobs. The company has provided materials to study as well as optional practice exams. Gloria and Lakeisha have signed up to take the first practice test. Reggie, however, jokes that he will probably wait until the night before the test to read the material. As the date of the test approaches, Reggie becomes increasingly nervous. He makes frequent comments about how hard it is for him to learn a different way of doing the jobs that he has been doing for years, joking that “you can’t teach an old man new tricks.”

End-of-life care becomes an issue at some point for elderly clients. Even with the emergence of palliative care programs and hospice programs, the majority of elderly people do not die in their own home as is their preference. What are the reasons for this trend? Discuss what you can do as a nurse to support your clients in end-of-life care and in supporting their desires. Support your response with evidence-based literature.

Most people consider home deaths as an outcome of high quality palliative care. The view of the home as the optimal place to die has been challenged, with the establishment of palliative care in hospitals, nursing homes, hospices and other in-patient facilities. Most patients wants die with the presence of their loved ones and be able to enjoy the last few moments with them. For many, not being able to die at home could therefore be seen as a failure in the care given to these patients, even if the patient was actually admitted to e.g. a hospital or hospice for perfectly valid reasons and in accordance with the patient’s wishes. There are many more reasons why most elderly patients prefer to die at home. With the emergence of home health care team, it encourages medical care to be continued at home while enhancing patient’s quality of life with their loved one. According to the article “Where do elderly patient’s prefer to die”, failure to provided death at home was mostly due to a prolonged terminal phase leading to emotional and physical exhaustion in the patient or the family (Groth-Juncker A., 2015).

In accordance to the American Nurses Association(ANA) statement position, it is the nurses duty to provide comprehensive and compassionate end-of-life care . Nurses are obligated to provide care that includes the promotion of comfort, relief of pain and other symptoms, and support for patients, families, and others close to the patient. Throughout this position statement, the term “family” includes those linked by biology or affection; family is whomever the patient says it is (ANA, 2016). In addition, it is also the nurses responsibility to discuss with the physician the patient’s wishes and incorporate social worker and pastoral care.

I currently work in Medical Surgical Oncology unit where I deal with death and dying quite often. There are many cancer patients who wants to die at home rather than at the hospitals, reason being they want to spend their last few days with their family. Most of these patients were offered hospice care, palliative or home health. Although the patient choses one of these care continuum care, it is possible to have it continued at home and they don’t have to stay at the hospital unless they do not have any family, home or anyone whom they can spend their last few moments with.

1-What are the reasons for elderly patients not dying at home as they desire?

Poor communication leading to unwanted care, increase in repeat hospitalizations, increase in late transitions, increase in ICU usage, fee-for-service care attracting financial gain, family members wanting interventions, medicare rules not allowing reimbursement for both skilled nursing facility care and hospice care.

2-Interventions/ activities/ referrals to support your clients in end-of-life care and in supporting their desires

Physicians, patients and the families should discuss end of life option and develop a plan early, Patients should complete an advanced directive and living will clearly stating their end of life choices, Patients should assign a health surrogate that is aware of their wishes, health care facilities need reform, medicare needs reform, patients need earlier referrals to hospice.

The aspect that I found most interesting in the reading is the polarity between the older and the newer nursing theories. The older nursing theories are associated with taxonomies and quantitative measures, whereas the newer more holistic nursing theories are more qualitative measures and softer phenomena (Shelly, J., 2006). The revolution in the approach of how we think about nursing and nursing interventions in association to ethical decision making relates to a belief that nursing as a profession is both  a science and an art-based profession, and the the new theories expand upon the concept. The other aspect that is interesting to me is using world views to challenge contemporary ethos by using philosophical clarity, religious sensitivity, and proportionate level of care for the sick and others who may need other forms of physiological and psychological interventions and/or wellbeing.

As medicine is traditionally considered a healing profession, and modern medicine claims legitimacy to heal through its scientific approach to medicine (Starr, P., 1982), the marriage of science and medicine has generally empowered nurses and physicians to intervene actively in the course of disease, to effect cures, to prevent illness, and to eradicate disease Hauerwas, S. (1990). In the wake of such success, nurses and physicians trained as biomedical scientists, have focused on the diagnosis, treatment, and prevention of disease. In the process, cure, not care, became the primary purpose of medicine; as nurses and physician’s role have transformed to the “curer of disease” rather than “healer of the sick (Starr, P. (1982). Thus, healing in a holistic sense has faded from medical attention and is rarely discussed in the medical literature.

The polarity between the older and newer nursing theories is the aspect which I have found to be the most interesting during my reading. From my reading, I have found that the older theories are a quantitative while, the newer ones are of qualitative measures.The revolution in the approach of how we consider nursing and nursing intercessions in relationship to basic moral leadership identifies with a conviction that nursing as a calling is both a science and a craftsmanship based calling, and the new speculations develop the idea (Puchalski, Blatt, Kogan, & Butler, 2014).The other perspective that is intriguing to me is utilizing world perspectives to challenge contemporary ethos by utilizing philosophical clearness, religious affectability, and proportionate level of watch over the wiped out and other people who may require different types of physiological and mental intercessions and additionally prosperity.

If you are trying to decide whether to use a z-test or a t-test, you are trying to determine how well your sample size represents the entire population. A larger sample size will better represent the entire population to a certain point because after a certain size, t and z distributions begin to look the same. If a sample size is to small it will be less representative of the entire population and it is more likely to either miss characteristics of the entire population or over emphasize characteristics. There are different critical values for distributions t and z (Foltz, 2012).

The z-test chart would be best to use with sample sizes over 30 and represents what is considered a normal distribution. If you want to know how many people in a school scored over 95 on a test and you know the average is 85 with a standard deviation of 15, and you take a sample size of 40 people, this would be a good time to use the z-test (Foltz, 2012).

The t-test chart would be best to use with smaller sample sizes and allows greater room for error. The distribution of the t-test curve is more platykurtic when compared to normal distribution or the curve for the z-test. If you wanted to know how many people in a school scored over 95 on a test and you know the average is 85 with a standard deviation of 15, and you take a sample size of 20 people, this would be a good time to use the t-test. The benefit of using the t-distribution is that you can use a smaller sample size with the understanding that there may be a greater margin of error (Foltz, 2012).

It may also be appropriate to use the t-test if the population standard deviation is unknown (Grove & Cipher, 2017).

Z-test and T-test are methods for hypothesis testing (which is the main goal of statistics). Hypothesis testing determines which statements about the population is most reliable based on data available.

For example, a hypothesis that claims those individuals who take Airborne regularly are less likely to become sick during influenza season than those who do not take the supplement. A hypothesis test can be done by giving sample (sample of 40 individuals for instance) doses of Airborne for a certain period of time compared to a control group.

Z-test is utilized for larger set of data or when sample size is greater than 30 (n ≥ 30). Based on the previous example, there were 40 individuals mentioned. Therefore, Z-test is more appropriate to use over T-test. Z-test is done to compare two groups when population variance (standard deviations) are known. Therefore, when standard deviation is known, z-core can be calculated. With normal distribution, researcher can determine probabilities using mean from the sample. This method allows researcher to compare scores from different normal distributions.

Sometimes, samples are small so T-tests are used. T-test is more applicable when participants are less than 30 (n ≤ 30). T-test is applicable when standard deviation is not known and when certain data sets are not suited for analysis using normal distribution. While z-test follows normal distribution, t-test follows t-distribution (or Student’s t-distribution).

Colleague 1: Aimee

A logic model would help us to understand the needs of the community and identify barriers to services. In looking at the needs of the community I reside in, one specific need that continues to arise is that of transportation. I live in a very rural area where public transportation is almost unheard of. There are no bus lines, taxis, or UBER. Many drivers for UBER or LIFT, will not drive out this far to aid individuals. Clients have difficulty accessing services unless they have a caseworker who is willing to transport, family in the area, or friends they can count on.

A logic model would help to identify the issue and create a plan to overcome the challenges faced with no public transportation. We could identify the need and potential solutions to the problem. Dudley tells us that “logic model provides an organizing framework for understanding evaluations” (Dudley, 2018, p. 169). IN looking at the lack of transportation, we may see a pattern in untreated mental health and substance abuse as having a correlation with a lack of transportation to appointments and no access to providers.

The short-term outcomes of public transportation would be greater access to services, such as; mental health counseling, drug and alcohol counseling, parenting, and individual counseling. The long-term outcomes would be lowered drug abuse, wider spread mental health treatment, extended revenue for the community as resources are being utilized, and the potential for lower mental health hospitalizations. Depending on how the intervention is implemented, will depend on the outcome (Dudley, 2016). The logic model, at the intervention stage, provides a framework to improve the interventions being used, (Dudley, 2016, p. 169). It allows the intervention to be focused on the unmet needs of the client and address potential success in outcomes, (Dudley, 2016).

A logic model is a diagram of the relationship between the need and the ways to address those needs.  It provides a concise, one-page picture of program operations from beginning to end. The four components of logic models are the problem statement – why the service or project is needed, the goal – the overall aim, assumptions – the basis for the ideas, and the rationale – information in the model (Randolph, 2010).

 

The theory of change typically shows the big picture including issues that can not be controlled.  Shows different avenues that can lead to change.  Describes the how and why the change can come about.  Explains the thought process behind the reasons the change will happen (Randolph, 2010).

 

Unmet needs are there is still a long waiting list of homeless Veterans looking for services.  There is also a proposed campus to be built at Veterans Village for women Veterans, but the cost of land, materials and labor has inflated due to increase in population.  Current donations and volunteers would need to double to get the fourth campus built within the allotted amount of time (Stalk, 2018).

Colleague 1: Sandra

An explanation of the types of skills the social work administrator demonstrated as she addressed the problem of Carla’s absence at work and the trauma-related events that followed.

The social work administration uses direct leadership style to take control of the situation. She gave instruction to the receptionist to wait another 15 minutes, after which she should apologize to the client, see if they would like to see someone else (if in crisis), and tell them that Carla would call to reschedule the appointment (Plummer, Makris,  & Brocksen, (Eds.). (2014b). She included what is expected of her, how it is to be done, and a timeline for when it should be completed (Northouse, 2013).

The Administration uses communication to manage conflict resolution by using the principled negotiation. This highlights deciding issues on their merits. According to Fisher and Ury, 1981, principled negotiation shows individuals how to obtain a fair share without taking advantage of.  There are four parts to this approach which includes separation of the people problem from,  focus on the interest, not positions,  invent options for mutual gains and insist on using objective criteria (Norhouse, 2013, pg., 251).

Finally, identify one aspect of the case study that would be most challenging to you if you were the administrator, and explain why.

One aspect of the case study that would be challenging is having to call the staff members to tell them that a fellow co-worker has passed. I would also feel compelled to tell them how she died so they could take steps to be mindful of their neighbors (if they are having any issues) and other people including the clients they served.

Colleague 2: Tammy

Post an explanation of the types of skills the social work administrator demonstrated as she addressed the problem of Carla’s absence at work and the trauma-related events that followed.

The social work administrator demonstrated good leadership skills in the handling of Carla’s absence.  The administrator used technical competence because she knew the order of the tasks that needed to be handled.  According to (Northouse, 2018) you should understand the intricacies of how the organization functions.  The administrator made sure that the clients were handled in the proper manner.  During my first field experience, the clients that showed up for a social worker who had called off was handled in this exact same manner and it was effective.  The administrator also had interpersonal skills because cared about the staff getting notified about the death of their co-worker in a timely manner and grief counseling was available the next day (Social work supervision: Trauma within agencies, 2014b).  (Northouse) asserts that having interpersonal skills allows leaders to work more effectively with that staff and supervisors.  The administrator also demonstrated emotional intelligence.  The administrator is in tune with her emotions and she is sensitive and aware of other peoples emotions (Northouse).

Be sure to include an analysis of the administrator’s use of conflict resolution skills.

One of the ways the administrator used conflict resolution skills was to separate the conflict from the person.  The conflict was that clients were not being seen because of an employee not showing up for work.  The administrator separated the two things and was able to handle each item separately.  The administrator also used fractionation when she realized all the elements that were involved in Carla missing, she divided the problems down into more manageable pieces (Northouse, 2018).

Finally, identify one aspect of the case study that would be most challenging to you if you were the administrator, and explain why.

One aspect of the case study that would be most challenging to me if I were the administrator is the task of notifying Carla’s clients that she had died suddenly (Social work supervision: Trauma within agencies, 2014b).  Sometimes, I find it difficult to find the right words when speaking about the sudden death of someone you know or work with.  This same scenario happened at the prison where I worked and when I read this story it brought back the memory.  One of the things I remember is asking someone which of our supervisors had made the call to the police because I wanted to commend the supervisor for doing the right thing.  During the time, no one spoke to the staff about the death of one of its employees.  If my memory is correct they might have mentioned it in our roll call and that was it.  I have always found death notifications to be particularly uncomfortable however, I would be able to complete the task if it was assigned to me.

Case Study #2

Claude Jean-Baptiste is recovering from post-hip replacement surgery and has been transferred to the Rehabilitation Institute adjacent to the hospital. When he enters the unit, he sees welcoming signs written in several languages including his own, Creole. Since there are no nurses on that shift that speak Creole, they use a language line to ask for translation services. During this initial nursing assessment, the translator informs Mr. Jean-Baptiste that the nurses invite him to have a relative at his side so that they can be sure to understand and meet his needs. He is asked about Haitian customs and beliefs that they might honor. Mr. Jean-Baptiste is encouraged to bring food and spiritual care items, and to share the warmth of his culture with the nursing staff.

Throughout this course, you have been exploring the ways in which social psychological principles can be applied to various careers. This week, you will create a literature review of articles that cover some of the main topics of applied social psychology. You should frame your review as if you were explaining the purpose of the field to someone who does not know about social psychology. To do this, you will write a literature review that contains five articles, each of which focuses on a different theory. Two of the five articles can be from the literature review for your case study that you wrote last week; however, they each must cover different theories. The other three articles should pertain to different theories that have been discussed throughout the course or that will be discussed in Week Six.

Remember, even though these articles are not necessarily related, the literature review must still have a thesis statement (e.g., Applied social psychology is a field that is important because it is relevant to…) and a conclusion that reinforces your claim. Since you are allowed to use your review of two articles from your previous literature review, please ensure you make any necessary modifications to your analysis so that they match the thesis statement and conclusion of this literature review.

The literature review must be five to seven pages in length, not including title and reference pages. All of the articles that you use must be recent (published within the past 10 years). Comment 1A business process is a collection of related activities that produce a desired outcome for the company that utilizes the process in order to deliver value to a customer. The business process that I will be using as an example is purchasing a product at Walmart.

   The triggering event is a customer purchasing a product at one of Walmart’s stores. Whenever a customer purchases an item, the customer inadvertently interacts with Walmart’s POS system. That information about the product purchased is handed off to the associated Walmart supplier. This supplier produces the product, labels it, and packs it for shipping. Information about the product’s destination is then handed off to an external shipping company. In the shipping activity, there are two outcomes (Claribel Slide 10).

           One outcome of the product is the shipment to a Walmart distribution center within 72 hours of ordering the good. This 72 hours could serve as a potential bottleneck. The product is received, selected, packed for shipping, and then reshipped within 48 hours of the product being received by the center. Within another 48 hours, the good is shipped, received by an individual store, and stored in preparation for selling. Eventually, it will be sold to a customer, who will start the process over. The 48-hour time limits mentioned above could prove to be potential bottlenecks. The second outcome of the shipping process is that the good is shipped directly from the manufacturer directly into storage at an individual store, where it will be sold to a customer to restart the process (Claribel Slide 10).

Variations could occur in the manufacturing activity of the process. Goods could be created with defects of other problems that could delay the rest of the process. This can be especially common in businesses that Walmart deals with, as one of Walmart’s business rules is to partner with manufacturers that can produce a good at the lowest possible cost. Variations could also occur in shipping, where a good may not reach the correct destination and delaying the process. The cost of this process is generally very low overall. Walmart is able to use its leverage as a large company to pressure suppliers into producing for minimal profit and at high volumes, potentially causing capacity restrictions.

Business Process: Freshmen student registration.

Since freshmen are new to the system at Widener University, there are several steps that must happen to ensure that the students are in the correct courses, in the correct sequence for progression. The School of Nursing (SON) curriculum is very rigid and does not have any free electives so there is no room for error. If a student takes a course that does not count for a required pre-requisite or general education requirement, they will be required to pay out of pocket for the course during the summer to stay on track. In addition, there are courses that have pre-requisites or minimum SAT/ACT math and/or English (reading scores). Students cannot take those courses without meeting the pre-requisites.

  1. Secretary sends email to home email address to remind students to set up campus cruiser email and access. She reminds students to sign up for one of six possible Pride Registration days.
  1. Secretary emails directions to configure computer to have access on campus, directions to access the online student planning system (through Campus Cruiser), a list of social science and humanities courses, a list of required pre-requisite and general education courses. Students are encouraged to go into the system and begin to build a schedule.
  1. The day of registration, the students receive a brief in person tutorial of how to access Campus Cruiser and Student Planning.
  1. Students who have taken AP/college courses document the courses on a form to be handed in to the faculty.
  1. Students log into campus cruiser and open student planning.
  1. Students choose classes based on curriculum and availability.
  1. Faculty and volunteer staff walk around room and assist students to build schedule.
  1. Faculty and volunteer staff confirm schedule before students register.
  1. Student registers.

Desired Outcome:

           Students register for the appropriate classes.

  1. Departments within the College of Arts and Sciences build courses for freshman general education and non-nursing pre-requisite courses and assigns faculty which are entered into Colleague.
  1. The SON builds freshman nursing courses and assigns faculty which are entered into Colleague.
  1. Once the courses are built, the registrar assigns rooms to the courses and they are published in Colleague for the students to see.
  1. Students then register for classes using the online system.
  1. Faculty and volunteers then confirm the schedule
  1. The secretaries in the SON check the schedules of all incoming freshman to ensure they are accurate.
  1. If there is an error, the Assistant Dean of Students corrects the schedule.
  1. The freshmen faculty advisors do a final check of the schedule on the Friday before classes start.

Business Rules:

  1. Students cannot register before they pay the deposit.
  1. Courses are created and approved by the department. No other department can create courses for a different department.
  1. All courses have a maximum capacity of seats.
  1. Pre-requisites must be met before a course can be taken.

Current freshman enrollment is 860 students. There must be enough classes for all 860 students to have the required courses needed.

Bottlenecks:

  1. Students, who do not go into Campus Cruiser and set up their email and Campus Cruiser access prior to registration, must do so during registration. Since the registration time is only 1.5 hours, they may miss the session after registration while they are trying to set up their access.
  1. Departments do not open up enough seats for registration. The students must then wait for the registrar to get permission to open new sections or raise caps.
  1. Wifi access can be an issue if a large number of students are trying to access it at the same time.
  1. Students, who have not reviewed the course selections list, do not know what they would like to take for social science/humanities electives.

Variation:

1. Students who come to Widener with AP courses or college courses, need a different schedule or a minor depending on the number of credits.

2. Students who have a math SAT score less than 530 or an ACT score less than 19, must take math 101 in the fall.  Math 101 or a SAT score 530 or greater or ACT score greater than 19 are required to take chemistry. This means that the student will take Chemistry in the spring semester freshman year and then take anatomy andn physiology I in the summer to get back on track with the science progression. 

Costs:

Prior to the online system, students completed paper registration with their top three choices and the registrar’s office built their schedule. On the Friday before classes started, many students changed their schedules, which required paper drop/add slips processed by the registrar’s office. The costs included paper (for forms) as well as manpower. The student’s schedules were still checked by the SON secretaries.

Since the institution of the online computer system, the registrar’s office has seen a decrease in the number of hours creating schedules and inputting drop/add forms. They do volunteer during student registration. Faculty volunteer their time to assist the students. There is the additional cost of the computer program for online registration but I do not know the actual cost
