For this module, you will be engaging in a few different necessary activities that “set the stage” for your first counseling session. Every initial meeting with a client requires an ethical discussion regarding full disclosure of the counseling process. Within this discussion, one should be acquiring a signed informed consent form. In this class, you will be required to practice your skills with others; ethically you are required to provide informed consent and to get their agreement in written form.

One of the most powerful foundational counseling skills is silence. It is important for counselors to be able to sit comfortably in silence while focusing fully on their client. The final part of this module’s assignment intends to help you identify how this might or might not be a challenging skill for you to develop.

Directions:

Follow the below directions to complete this three-part assignment. First, you are to create a counseling informed consent form and an opening statement. Then you are to identify someone to assist you with a short “counseling” session where you will practice your informed consent and opening statement as well as the “Five Minutes of Silence” exercise. You will then write a reflection critiquing your experiences.

Part I: Develop an Informed Consent to Counseling Form

Review the “Written Consent Template.” Develop your personal written informed consent form referencing the template as a guide. Your form should have a personal letterhead, contained to a single page (you can use single spacing), and contain all of the required essential information.

There are five major sections for your consent form: Letterhead, Introduction, Confidentiality, Audio/Videotape, and Boundaries of Competence (see the template above). Note that your informed consent will vary according to the setting (i.e., a mental health clinic, a psychiatric hospital, or a private practice) and the client with whom you are working (i.e., a child, an adult, or a couple).

Part II: Create an Opening Statement(Foundations: Counseling Silence Exploration)

In approximately 1 page:

Create an “Opening Statement” monologue regarding what you might specifically say to your client when meeting him or her for the first time. This statement should include the following:

  • Formal education/training
  • Continuing education and supervision requirements
  • Your supervisor’s information
  • Philosophy and approach to counseling (write in layman’s terms)
  • Counseling fee schedule—Remember, for your practice sessions there will be no fee!
  • Limits of confidentiality (also review the American Counseling Association’s [ACA’s] website)
  • Contact information for any complaints related to counseling/counselor

Part III: Practice and Reflection of Your Informed Consent, Opening Statement, and Five Minutes of Silence

Part of this module’s assignment is to practice your opening statement with a “client” and review your informed consent. For this session, your informed consent should include informing your client about the silence exercise. The “Five Minutes of Silence” exercise will include two parts: 1) where you, as the counselor, attempt to stay silent for five minutes allowing the client to speak while practicing being attentive; 2) where you sit in complete silence with the client for 15 seconds.

It is important for clinicians to be comfortable in silence and be self-aware of their nonverbal attending skills, thus allowing clients to have time to reflect during the session and choose to speak when ready. Nonverbal body language can also say a lot to the observer as to whether a counselor is fully engaged and attentive. Additionally, it is important to be comfortable with absolute silence as this can give client’s time and permission to internally process.

For this “Five Minutes of Silence” portion of the assignment, you will take an opportunity to practice both skills. You are to engage with your client, during which you must take at least five minutes of this time to say absolutely nothing but try to just fully understand what your client is saying and feeling. Additionally, you will practice sitting in silence (both of you being silent) for at least 15 seconds to explore your reactions to the silence.

SOLER Technique

While listening to your subject, be aware of your own nonverbal attending skills. Effective listening is the key to being an effective counselor. These nonverbal skills are covered with the acronym SOLER (Egan, 2014). (Note: Use of this resource is not necessary to complete the activity.) Egan (2014) presents “SOLER” as a way the listener can: “. . . make sure you are physically present to a client.”

  • S—If it suits them, face the client squarely (some prefer up to 45 degrees, etc.).
  • O—Maintain an open posture with the client.
  • L—Lean toward the client (as appropriate) and nod your head.
  • E—Maintain appropriate eye contact with the client. Always consider culture when maintaining eye contact and watch for signs of stress or discomfort. (There is also a suggestion of changing the “E” to “A” to symbolize “aim” toward the client physically and directly, for the visually impaired counselors.)
  • R—Be a relaxed helper, as by doing so you greatly improve the quality and comfort of the sessions.

Reflection:

In 2–3 pages, critique your delivery of the informed consent, opening statement, and nonverbal attending skills (i.e., visual/eye contact, vocal qualities, verbal tracking, and body language—SOLER). Include the following in your critique:

  • A critique of your opening statement and informed consent delivery
  • A statement regarding if you were able to meet the minimum of 5 minutes of attentive silence or if you had difficulties
  • Identification of what was getting in the way of you fully listening for understanding while remaining silent
  • Discussion regarding your internalized thought and emotional process
  • Determination of what else you were aware of during this time
  • Explanation of how your “client” reacted to your silence
  • Development of strategies of ways to maintain this attentive “self-silence” in the future

Allow yourself at least three attempts to get the full five minutes, and then you can stop trying.

Your final product will be in a Microsoft Word document, written in APA format, and be approximately 3–4 pages in length (including your consent to counseling form). Your paper should be written in a clear, concise, and organized manner; demonstrate ethical scholarship in accurate representation and attribution of sources; and display accurate spelling, grammar, and punctuation.

Client reports that his drinking has increased over the past couple weeks and that he is utilizing alcohol as a way to cope with the current problem. He states that he is drinking 2 to 3 vodka and tonics nightly and that this amount is unusual but it is one of the ways that he is able to sleep and not have to think about what she did. Before finding out about this incident, client reports drinking one to two drinks a week. Client reports no prior problems with alcohol but he does report binge drinking with fraternity buddies while in college. Reports this stopped soon after he accepted a full-time engineering position. Client reports no current or past drug use. Does report trying marijuana while in college but stating he didn’t like it. Client is a non-smoker. Client reports no other general medical concerns.  Client and wife have owned their home for the past 10 years and are satisfied with their living conditions. Client reports that he is attending counseling today because his wife made him. Client reports a long history of porn watching beginning in childhood. States that he thinks his family dynamic was ‘fine and that there weren’t any problems when I was growing up’. Does report that his mom and dad were distant and very conservative. Reports they treated him like a child even after he became an adult. States he had many rules he had to follow as a young boy so when he was tired of following the rule he just hid what he was doing from them. States his mom used to say that it was her job to make sure he turned out right and he always felt like it was his job ‘to make sure she didn’t find out what I was up to’. States that he has two younger brothers and his relationship with them has always been fine. States that he attends ball games with them and sometimes watches Monday night football at the bar with them. States him and his brothers still have dinner with his parents every Sunday.  Client reports he has been married to his current wife for 25 years. Reports that the relationship has had ‘ups and downs’ over this time but that it is generally ‘allright’. States that it is currently ‘down’ because of her knowledge about the pornography use. Client states that he has worked as a journeyman tool maker for the past 33 years and a satisfied with his job. Reports military service for ‘four years and then out’ completing when he was 23. Client states that he makes enough money but always could use more. Client reports that he was recently diagnosed with high blood pressure and has started taking medication for this. Client is a smoker and reports currently smoking about half a pack a day. Client states he has never had any mental problems but he does report that he and his wife went to counseling about a year ago for three or four sessions. Client 3 Case  Client is a 22 year old female. She states she has come to counseling because she wants to do a better job knowing herself and be brave enough to let others know her too. Client reports that she came out as bisexual several months ago and has had mixed reactions to this announcement. She states that she has told a few friends and some of them have disowned her. She reports that she did not do anything about this in high school and ignored how she felt because she was worried about what others would say. 1.  What is the title of the article? Provide the reference for the article in proper APA format.

2.  What is the purpose of the article?

3.  What is the hypothesis of the study? In other words, what claims do the authors make in the article?

Substance abuse counseling lends itself nicely to the integration of positive psychology and spirituality. While both of these therapy approaches have been explored for many years, the integration into substance abuse counseling is a relatively new development. As such, many clients may not be aware of the techniques in positive psychology and spirituality that can be of use in their recovery.

Juanita has been seeking treatment for her marijuana use from you for a month now. While she has significantly cut back on her substance use, she is having difficulty quitting and remaining abstinent for periods that last longer than a few days. In your counseling sessions, Juanita has mentioned that she feels very badly about herself—she feels guilty about being unable to quit and explains that she doesn’t think she is strong enough to quit altogether. She has also expressed nervousness that her boyfriend would no longer like her if she quit smoking, and she has expressed concern that if he leaves her, she wouldn’t be able to find a new boyfriend because she doesn’t believe she is pretty enough. Juanita is also unsatisfied with her job as a cashier at the local grocery store, but has expressed her opinion that she doesn’t think she is smart enough to get a better job. You know from Juanita’s history, that her mother was very critical of Juanita when she was a child and that her father was absent for most of her childhood. In high school, Juanita excelled at classes in biology and math, but she never attended college.

To help Juanita overcome her barriers to marijuana abstinence, write a summary of what the counselor should cover in the next meeting with Juanita. Ensure that you cover the following points:

  • Explain the concepts of positive psychology to Juanita.
  • Examine the goals of positive psychology and explain how this approach will help Juanita obtain sobriety, focusing specifically on the concerns and dissatisfactions she has expressed.
  • Design at least two exercises based on positive psychology for Juanita to work on in between counseling sessions.
  • Explain spirituality to Juanita, emphasizing the differences between spirituality and religion.
  • Examine how spirituality and positive psychology can work together to help Juanita achieve her goals.
