Based on the readings assigned and suggested for this week and a review of the seven-stage, sequential framework to promote greater use of EBPH, do the following:

The application of evidence-based principles to a public health issue demands a scientific quantification of said public health issue. An integral component of the community assessment should include the targeted health condition or risk factor being considered, the population affected, the size and scope of the problem, prevention opportunities, and potential stakeholders. To aid in this task, epidemiology and the US surveillance system are applied to track and obtain necessary information about the frequency of the health condition or risk factor in an affected population.

Although data from surveillance systems can be used to obtain baseline and follow-up measurements for target populations, there may be limitations when using the data to evaluate intervention effectiveness for narrowly defined populations. In such a case, it may be necessary to estimate the frequency of disease or other health condition for the target population by using special surveys or appropriate study designs. Understanding the tradeoffs of various study designs will improve how you evaluate the effects of various public health programs and policies.

Review the study designs from the readings assigned and suggested for this week and present a brief summary of the appropriateness and advantages of using each study design as part of your effort to quantify the public health issue that you identified in Week 1 as part of your community assessment.

Write your initial response in approximately 300 words.

By the due date assigned, post your response to the appropriate Discussion Area. Through the end of the week, review and comment on at least two peers’ responses. Consider commenting on their stated advantages of using a specific study design. Do you agree or disagree with their stated advantages and why?

Support your answers with examples and research. Your responses should clarify your understanding of the topic. They should be your own, original, and free from plagiarism. Follow the APA format for writing style, spelling and grammar, and citation of sources.

Complete the following:

  • Create a title page: Write a brief title that concisely conveys the purpose of this report. It is suggested that you use the APA Paper Template, linked in the Resources under the Suggested Resources heading, to format your paper to format your work according to the APA manual (sixth edition).

  • Create a heading titled Theoretical Framework: Analyze how two or three psychological theories help explain the problem and point toward potential solutions. In doing so, cite relevant principles or concepts derived from these theories. For example:
    • Let us say that we are assisting an adolescent from Germany who has moved to United States, and is having a rough time adjusting to her new high school. In addition to information gathered from her, her parents, and the school, we could consider Erikson’s psychosocial theory and the stage of identity and role confusion in which adolescents search to establish their own values, beliefs, and goals. During this important time of identity development, the teen has been immersed in an entirely different culture with values and beliefs different from the one with which she knows. We could consider to what extent she was mastering the stage prior to and then after the move. Based on sociocultural theory, we would enquire about the differences in culture that may be impinging on her success in interacting in her new environment. By integrating the two theories, we can get a better understanding of how her developmental stage is clashing with her sociocultural context. We might search for research on adolescent development and cross-cultural experiences to get a better understanding of what research demonstrates is effective in assisting teenagers in this position.
  • Create a heading titled Related Research: Summarize at least three research studies that pertain to the problem. A summary will provide your reader with a clear understanding of the purpose of the study, the methods, and the findings.
    • Describe the research designs and methods used in the studies.
    • Use principles of critical thinking to evaluate the quality of the research and analyze the relevance to the problem.
      • Evaluate how the research methods employed support the studies’ findings.
      • Analyze how the studies’ findings explain reasons for the problem and point toward potential solutions.
      • Analyze how the studies’ findings relate to the psychological theories you previously cited.
      • Compare findings of the studies and assess how they support or contradict each other.
      • See if any contradiction is apparent. If so, try to explain why the contradiction may have developed and which position is more credible and why.
  • Create a heading titled Action Plan: Based on your theoretical and research findings as well ethical and cultural considerations, provide an action plan with recommendations for how to resolve the problem you researched in a fair and just manner.
    • Make clear how the objectives of your plan are specific, measurable, achievable, and relevant to the problem, and the timeline for implementation should be.
    • Integrate information from your sources to support your recommendations.
  • Create a heading titled Conclusion: Briefly summarize the importance of the problem, how psychological theories, principles, and research may point to ways improve the problem, and the implications for a larger societal problem or problems.

Maria, a 70-year-old Hispanic woman, lives with her daughter, son-in-law, and granddaughter in a spacious home on the lake. She enjoys reading on the patio, walking along the river’s edge, and taking her granddaughter to the park. She is fluent in Spanish, yet understands and speaks a little English.

Angel, Maria’s daughter, has noticed some behavioral changes in her mom that are troublesome. Her mom fell while walking to the car before they left for the restaurant and she had fallen the week before when walking into living room. Sara, Maria’s granddaughter, told her mom that she had to hold Grandma’s hand and bring her home from the park because she forgot how to get home.

Angel called the doctor and scheduled an appointment for her mom. Once Maria learned her daughter was planning to take her to the doctor Friday, she was very upset because she hadn’t been consulted first. Also, Maria didn’t have any bothersome symptoms and decided there was no reason a doctor needed to see her. All she needed was more sleep.

Maria and Angel arrived at the doctor’s office early Friday morning. The nurse brought them back to the examination room upon arrival as they were the first appointment of the day. Displeased, Maria allowed the nurse to take her weight, pulse, and blood pressure. Once the nurse was finished, she left the room and the doctor came in. While asking Maria some questions, the doctor spoke slowly and loudly as if she couldn’t hear. After the doctor learned that Maria’s understanding and speaking of English was minimal, he talked to Angel the entire time and then ordered additional tests without Maria’s consent. Maria was furious and wanted to go home.

Consider the individual, family, cultural beliefs and expectations, and ethical issues the individuals must think about as they interact with each other.

Fibrocystic change is categorized as a benign breast problem. Symptoms of fibrocystic change include breast tenderness, swelling, pain, or irregular menstrual cycles (Arslan Solmaz, 2018). Patients can feel lumps in their breasts. Movable lumps filled up with fluid are called cysts. The patients can find their breasts swollen or tender to touch. It happens when breast tissues become firm and rubbery like scar tissues. Fibrocystic change occurs mostly in women of child-bearing age, but they can affect any women (American Cancer Society, 2017). The changes in hormone related to menstruation can cause the lumps to get bigger and tenderer. Therefore, the symptoms are more noticeable just before the menstrual period starts. The patients may also feel a clear or cloudy nipple discharge. Hormonal abnormalities such as an excess of estrogen over progesterone or increased thyroid hormone activity are reported to be the cause of the fibrocystic change (Arslan Solmaz, 2018). A diagnosis is based on symptoms, history, and physical examination. A mammogram, ultrasound, or a biopsy will be needed to rule out cancer.

Treatment for fibrocystic change is necessary if the cysts cause discomfort. Medications include danazol, tamoxifen, hormonal therapies, or vitamin E (Talaei, Moradi, & Rafiei, 2017). The authors also mention that Metformin can help reduce the symptoms of fibrocystic change. Besides that, patients who take Metformin have a reduced mortality rate of cancer due to the medication’s antitumoral properties (Talaei, Moradi, & Rafiei, 2017). Another option is to have the cysts drained to reduce the pressure and pain, but the fluid can build up again (American Cancer Society, 2017). If that happens, the patients can choose surgery to remove the cysts. They can take pain medications such as ibuprofen or acetaminophen for pain relief as well. They need to avoid caffeinated drinks such as coffee, tea, sodas, or chocolate since they can stimulate the symptoms (American Cancer Society, 2017). Fibrocystic breast is considered to be a non-proliferative lesion. It generally affects women between the ages of 25 and 50 and rarely affects women under age 20 (Arslan Solmaz, 2018). According to the Center for Disease Control (2018), fibrocystic breasts can have symptoms of feeling lumpy, tender to touch and sore and have cysts that are fluid filled sacs within the breast. Patient education would include that fibrocystic breast disease is usually benign and and may be due to hormonal imbalance. Follow up care would include sonography, mammography and possibly biopsy to ensure that the cysts are not cancerous (Talaei, Moradi and Rafiei, 2017). Treatments can include a fine needle aspiration to drain the cysts, or surgical incision. Over the counter Tylenol and Ibprofen can be used for pain control. Oral contraceptives can be taken for women to lower hormone levels associated with breast changes (Talaei, Moradi and Rafiei, 2017). Self management would include pain management, breast self exams, warm compresses and monitoring any cysts for and change in texture, size and if they are causing further pain.

·  Begin with your end goal of your course of study in mind and summarize a Mastery Journey goal for each course in your program by providing the course name and at least one insight, skill, or outcome that you would like to gain from that course.

·  In developing your strategies/tactics evaluate the course content, include self study topics, library resources, and online tutorials (e.g. Lynda.com).

·  Research and include at least one Full Sail Library multi-media resource and/or tutorial per course that will assist you with your Mastery Journey.

·  Identify and provide the email/phone contact information for at least 6 industry

leaders that you could interview for a Mastery Journal blog post.

·  Include plans to get involved in your field of interest through clubs and organizations. Include how you will expand upon your PLN by following innovative companies and your heroes.

·  Include how you will get involved with the Full Sail University community (e.g.clubs, industry organizations, nonprofits, etc.) to get “real world” experience. Outline the characteristics of a mentor in your field so that you can set out to find that person.

·  Summarize your Mastery journey timeline key points and include a visual representation of your 12-month timeline depicting your Mastery Journey.
