Choose ONE of the cases listed below and create a mental health assessment for your chosen case. In the assessment, you should address the following:

·  Chose ONE of these cases to analyze.

1. Tony sometimes seems very wound up. At those times, he seems full of energy, talks very rapidly, and makes very grandiose plans. Once, he gave away all of his belongings and was planning to move to Washington, D.C., so that he could advise the President. At these times, he also seems to need almost no sleep. During other periods of time Tony seems very down. During these times, he does not take care of himself. He seems to want to sleep all the time, and he often makes thinly veiled references to wanting to commit suicide.

2. Paula has not left her house for several months. When she tries to go out, she experiences great anxiety. She says she is afraid that if she leaves her house to go somewhere, she will not be able to get back. Before all this started, Paula seemed fairly normal except for having several episodes where, for no apparent reason, her heart started pounding, she started to sweat profusely, and she experienced all the symptoms of fear and terror. One of the reasons she is afraid to leave the house is because she is afraid she will have one of these episodes again. Hint: there are two specific diagnoses here.

3. Horace sleeps a lot, has great difficulty getting out of bed in the morning, and generally does not want to do anything. He has stopped seeing friends whom he used to see often and declines all invitations to do things socially. His most common response is “I just do not feel like it.” He looks sad all the time and does not seem to take pleasure in everyday activities. This has been going on for the past two months.

4. Patricia is a 44-year-old female who reports that she has periods of time where she cannot remember what she has done. She reports that after one such period, she received a telephone call from a man who claimed to have met her in a bar where she was “the life of the party.” She had also told the man her name was Priscilla. Patricia thinks that this is odd because she does not drink, and she is a rather shy and retiring person. However, the man had her correct telephone number and was able to give a good physical description of her.

5. Frank was attending college in San Francisco during a recent earthquake. He lived in the area that was hardest hit by the quake. Frank was not home when the earthquake hit and was not injured in any way, but when he returned home, he found his building demolished and his two roommates crushed to death. Frank immediately drove himself to the airport, bought a ticket to Boston, and got on the plane. His parents found him on their doorstep in Boston the next morning. Frank remembers nothing about the earthquake and nothing about going to college in San Francisco. The last thing he remembers is being a high school student and living with his parents in Boston.

Think about ways you might apply a systems perspective to his case. Also, consider the significance of the systems perspective for social work in general.

Forensic psychologists are important advisors to the lawyers,  mediators, and judges in child custody disputes. Unfortunately, messy  child-custody disputes are an all-too-common occurrence when parents  divorce. In some especially difficult cases, one parent may make an  allegation against the other, and sometimes these allegations are made  primarily to gain the support of the courts.

As impartial evaluators, forensic psychologists provide psychological  assessments with a focus on parenting. The psychologist informs the  fact-finder (usually the judge). Because parenting evaluations are  supposed to be impartial by nature, they cannot involve themselves  directly in the child-custody hearings. In other words, the forensic  psychologist cannot approach the evaluation with any investment in the  outcome of the case.

In the case of child abuse and/or neglect, the state must often  intervene to provide a safe environment for the child until they can  either be safely reunited with their parents or they are placed in a  safe living environment. In these cases, forensic psychologists can  offer thorough evaluations to help identify risk factors for child  mistreatment, as well as ways to ameliorate risk.

Parenting evaluations consist of four major components:

1. Interview: Whenever possible, the psychologist must obtain personal history from the person being evaluated.

2. Record review (or review of “discovery”): The  psychologist can receive records from a number of parties to the case  and can use prior findings to help formulate a case.

3. Collateral consultation: Is the parent involved  with a counselor or a parenting agency? If so, it is usually helpful to  conduct a collateral interview with that service provider to ascertain  the parent’s progress in treatment and what further treatments might be  necessary.

4. Psychological testing: Testing is used to help  determine if there are underlying risk factors (such as mental illness  or low IQ) that might be contributing to the risk to the child.

In all evaluations, the “best interest” of the child is the chief  criterion for the judge. A best interest determination can be  strengthened with a solid psychological evaluation of a parent.

Patient HM has a history of atrial fibrillation and a transient ischemic attack (TIA). The patient has been diagnosed with type 2 diabetes, hypertension, hyperlipidemia and ischemic heart disease. Drugs currently prescribed include the following:

· Warfarin 5 mg daily MWF and 2.5 mg daily T, TH, Sat, Sun

· Aspirin 81 mg daily

· Metformin 1000 mg po bid

· Glyburide 10 mg bid

· Atenolol 100 mg po daily

· Motrin 200 mg 1–3 tablets every 6 hours as needed for pain

Case Study 3:

Patient CB has a history of strokes. The patient has been diagnosed with type 2 diabetes, hypertension, and hyperlipidemia. Drugs currently prescribed include the following:

· Glipizide 10 mg po daily

· HCTZ 25 mg daily

· Atenolol 25 mg po daily

· Hydralazine 25 mg qid

· Simvastatin 80 mg daily

· Verapamil 180 mg CD daily

To prepare:

· Review this week’s media presentation on hypertension and hyperlipidemia, as well as Chapters 19 and 20 of the Arcangelo and Peterson text.

  1. Create an itemized list of at least ten (10) items that you will need for your graduation party. Lay out your Excel spreadsheet following the example below. Make your columns as wide as you need to show all the information for that column by double clicking on the line between the columns or dragging the line between the columns. Cell B2 had the Wrap Text on the Home ribbon in the Alignment group turned on due to the length of the description. Only include numbers in the Cost per Unit, Quantity, and Cost columns so your calculations will work.

  2. When you have entered your data, Sort your information by Item.
  3. Share any questions about this or anything else in the Discussion Area.

MS Excel is a great help with calculations which are completed using formulas. Remember, by typing an equal sign (=) into a cell, you are preparing Microsoft Excel to do a formula calculation. Refresh your memory on the use of formulas from this week’s assigned reading and the online lectures. For Sheet 2, think of a situation at home, work, or even a hobby or sport for which you could use a calculation and create a formula to solve a problem.

  1. Open a spreadsheet in MS Excel and create your formula. Make sure you label components of your spreadsheet.
  2. In the Discussion Area when you attach your file, describe the purpose of your formula and how it will help solve the problem it was designed to address. Share any challenges you had as well as tips for others.
  3. Delete the unused worksheets by right clicking on them one at a time and choosing Delete
