She starts by defining the word “caring.” Within the last ten years she has refined her terminology changing the term caring to caritas.

This demonstrates a growth and maturation from decades of research she and other scholars did to produce the theory of caring. The third stage is theory testing. Defined as the phenomena behind the theory that are exposed through research. The definitions of terms are refined. The theorists and other researchers consider whether this theory helps answer questions that arise in practice. This is where your theory is used in by a widening group of researchers. For example, graduate nursing students request the tool you developed while testing your theory.

The fourth and last stage is evaluation. This is where philosophical debate occurs as the concepts are applied through evidenced based practice in the act of providing nursing care. Both Sam and Gloria graduated from the #1 ranked MBA program in the country and bring a lot of corporate experience to the business. Sam was a Senior Vice President (SVP) in Marketing and Sales at another company for many years before deciding to venture off to start ACHs with his wife. Gloria also worked for that same company and held the position of Finance Director.

The company is known for its own brand of gourmet coffee and low fat donuts, as well as for their own line of stainless steel coffee mugs that are able to keep drinks hot for 12 hours straight. The workforce is comprised mostly of bakers, managers, baristas, greeters, clerks, and cashiers. They also work closely with several large suppliers and distributors in the United States.

Problem

The company has a loyal customer base throughout Chicago and is known for its personalized service and quality food and drinks. Margaret Newman’s theory, Health as Expanding Consciousness, “was stimulated by concern for those for whom health as the absence of disease or disability is simply not possible” (Nursing theory, 2016). Newman’s theory “asserts that every person in every situation, no matter how disordered and hopeless it seems, is part of the universal process of expanding consciousness” (Nursing theory, 2016). Newman expressed that removing pathology will not change the pattern of the individual pattern (Nursing theory, 2016).

Newman expressed that “nurse client relationships often begin during periods of disruption, uncertainty, and unpredictability in patients’ lives” (Smith & Parker, 2015, p. 288). Majority of the time patients who are in the hospital are there due to unexpected events that occurred and now they are hospitalized. Some patients suffer such traumatic injuries and develop lifelong complications that will affect everything and everyone around them. As nurses, we tend to evaluate each and every patients situation, and at times we are more concerned and relate to the patients situation. Working in a rehabilitation hospital, I use theory very often, I would say probably on a day to day basis in my practice.

There are many stories I could share with the experiences I have had with some patients, however, I am going to share my experience I had with a patient that stayed in the hospital for about 2 months. He was a 66 year Old man, who had fallen off a tree, while trying to get the family cat from the tree, and suffered a major spinal cord injury. He was paralyzed in his lower extremities with very limited movement in his upper extremity. He spent a lot of time in an acute hospital, before he came to our facility for rehab. Each day I spent taking care of him, I learned something new about him every day. I learned about his family, what he did for a living, about his kids, and soon we developed a relationship where we would joke around and laugh and talk about his progress in therapies. He was a very sweet and kind man. He was the provider of his family, he was independent and was just not ready to give up. His wife and daughter came to visit him every day. Even though he joked around and was happy we knew that deep down inside he was suffering.

The day case management met with the family and patient about discharge planning, the wife told them she would not be able to take care of him, he needs a nursing home. The tears just rolled down the patient’s eyes. The process of discharge continued because the patient did not have the best insurance so finding placement was not very easy, but our case managers were wonderful and kept getting him more time in the rehab hospital. From nursing, to therapy, to case management, and almost every department knew his story and everyone just adored and loved him. The wife and daughter started to come visit less and less, and the one day the patient just broke down and said “I am useless, I am burden to my family, I cannot even take care of myself, I don’t blame my wife, I am nothing but a body, I just wish I would die”. I had to hold back tears. This is the person who provided for his whole family, and now that same family is unable to provide for him. As days went on he became less active, more quiet, depressed and ill. He stopped eating and drinking, he just wasn’t himself.

One day I went into change him, and we talked about therapy and the weather, my nursing tech helped me roll him over, and all of the sudden she said “I don’t think he is breathing”. His eyes rolled to the back of his head, we called a code and CPR was started and 911 was called. He had a PE, went into cardiac arrest and never came back. One of the saddest experiences of my life and probably of all the staff that had the pleasure of taking care of such a special man. As a healthcare team, we provided him with the best care, he developed relationships with all his nurses, nursing techs, therapist and doctors.

We build relationships with majority of our patients. Sometimes we are the only family these patients have. When life throws unexpected twists and turns at them, they are scared, angry and feel hopeless, and they turn to us for answers and that little bit of hope and confidence. I can say with confident that this patient knew and trusted us, even though he knew he would not be the same as before.

Health care information technology is a broad term that describes the technology and infrastructure used to record, analyze, and share patient health data. Health information technology was created in order to provide better care for patients along with the ability to help achieve health equity. Health information technology is able to help support recordings of patient data to improve healthcare delivery and allow for analysis of this information for both healthcare practitioners and ministry of health and government agencies. This data is used for the implementation of policies in order to better treat and prevent the spread of diseases. Health information technology improves the quality of healthcare delivery, increases patient safety, decreases medical errors, and strengthens the interaction between patients and healthcare providers (OpenMRS, 2015).

Data mining is defined as the computer-assisted process of digging through and analyzing enormous sets of data and then extracting the meaning of data. With data mining, business organizations, such as health care facilities, are able to make more precise business decisions which ultimately allow the facility to gain more profits. Data mining holds great potential to improve health systems. It uses data and analytics to identify best practices that improve care and reduce costs allowing for an increase in the quality of patient health care (Rajkumar, 2015).

Quality Performance Indicators

One important quality performance indicator that can be monitored is the length of time patients must wait before being seen by a health care provider in the ER. It is a measure of performance and efficiency of care. ER wait times are a vital measure of performance and quality of care as they track one of the most sensitive areas of a hospital. Hospitals must constantly strive to reduce wait times in order to offer better procedures for quicker treatment. Measuring how long patients must wait reveals issues in the admissions process, the speed of treatment, and health care providers’ ability to provide quality care. Additionally, they can be measured in both immediate terms, or over longer periods of time to determine trends and allocate resources better to reduce workloads and stress. Data is collected by using information technology from admission soft ware and patient records and the facility is able to measure overall ER wait times (Farquhar, 2016). By using this data, the facility is able to see where improvements can be made in order to increase patient satisfaction and decrease overall waiting room times.

Another example of a quality performance indicator is the rates at which patients fall and incur injury during a hospital stay. Between 700,000 and 1 million patients suffer a fall in U.S. hospitals each year, according to the Agency for Healthcare Research and Quality. By tracking falls using information technology and data to chart where, when, and with whom falls are occurring, health care facilities are able to see exactly where they need to alter and tailor the health care facility to help prevent future falls. In the end, this would lead to improved patient care outcomes.

Information technology and data mining both have an impact on the quality of health care provided to the patient. Information technology allows the advanced practice nurse to research the best practice suggested for a diagnosis. It also is used to find available resources to help narrow down a proper diagnosis or to assist with educational material to provide to a patient so they can better understand their disease process or medications being prescribed. These are all examples of how the quality of health care being delivered is increased with information technology (Melnyk & Fineout-Overholt, 2014).

Data mining is examining a large set of data and finding patterns that exist. This information is then used to predict if the same patterns are likely to occur in the future, and if so, how often they will occur. This process is much easier and quicker now that electronic health records are so popular. Data mining impacts the quality of health care by providing accurate reports over a period of time that can be used to examine best practice trends (Hamric, Spross, Hanson, & Hamric, 2009).

One quality performance indicator well known in the health care field is patient satisfaction. Measuring patient satisfaction helps the company be more aware of their shortcomings and provides insight into what adjustments need to be made in order to assure high quality care is being provided. In order to maintain high patient satisfaction scores, employees must consistently provide excellent care to the patients. By doing so, this changes care outcomes for the better. Not only does the patient benefit from this but the organization does as well. One benefit to the organization is an increase in business due to being well known in the community for having a good reputation. Another benefit to the organization is a decrease in the amount of readmissions leading to better reimbursement percentages. Although there are benefits to the organization, studies have shown that this may not necessarily be of greatest benefit to the patient. In one article, high satisfaction scores led to a 44% increase in mortality rate (“Patient satisfaction: When a doctor’s judgment risks a poor rating – amednews.com”, 2016). Analyzing patient satisfaction data with information technology allows users to evaluate what is really best for the patient.

Another quality performance indicator used in healthcare is lab turnaround times. This has been shown to be extremely beneficial to the patient because the faster the results, the faster the patient gets the care they need. In contrast, this can also lead to unrealistic expectations by the healthcare team. Communication can be affected and crucial things can be missed. This also leads to a heavy reliance on technology which has been known to fail without notice. Measuring lab turnaround times also put increased pressure on the lab personnel which can lead to an increase in errors due to unrealistic expectations. Advancements in technology have allowed wireless uploading of lab results leading to an increase in turnaround times which has been known to have an effect on patient outcomes.
