CF is a sixty-year-old African American male who presents as a new patient for initial evaluation and follow-up. He has been diagnosed with hypertension for the last twelve years and Type 2 diabetes for the same period of time. His current blood pressure is 162/90, with a pulse of 76. His body mass index (BMI) is 32. He is currently taking Maxzide 37.5/25 mg every morning. This is the only antihypertensive medication he has taken.

For this question, focus on the treatment of hypertension. Discuss the influences of his age, gender, and ethnicity on hypertensive medications. On the basis of an analysis of those factors, provide one option for improvement of his blood pressure and provide a clear and specific justification for that choice. Be sure to include dosage and scheduling. Include highlights of patient teaching and/or recommendations for any lifestyle changes. Support your decisions with at least one reference to a published clinical guideline and one peer-reviewed publication.

High blood pressure is a life-threatening condition where the blood’s long-term force on the walls of your artery is powerful enough to cause some heart disease (Go et al., 2014). It depends on the amount of the blood pumped by your heart and the resistance this blood encounters in your arteries. The narrower the arteries are, the higher the blood pressure. High blood pressure develops over the years. Fortunately, it is detectable and controllable. High blood pressure that is uncontrolled for a long time causes serious health problems such as heart attack, kidney failure, heart failure and stroke. However, hypertensive medication is influenced by the patient’s age, his or her gender, and ethnicity. Lifestyle changes boost the control and treatment of high blood pressure (Woo & Wynne, 2012).

The ideal blood pressure for people aged 60 years and above is 150/90. CF’s blood pressure is 162/90 which means it is high despite his daily intake of Maxzide 37.5/25. The ideal body mass (IBM) for an adult is between 18 and 24. CF is 32 meaning he is obese.Studies reveal that high blood pressure affects men more than women (Go et al., 2014). After the age of 55 years, women tend to suffer from blood pressure more than men.However, more male suffers from complications related to blood pressure more than females (Liao, at el., 2016). Most males with high blood pressure suffer from heart attack, heart failure, and other heart diseases if not controlled. Studies show that fewer people aged 39 years and below suffer from high blood pressure as compared to above 40. It is therefore correct to conclude that age influences the presence of high blood pressure. Several types of research have revealed that if one comes from a family history that has had blood pressure, there is a likelihood that they too are at an elevated risk of having high blood pressure at one time in their life. It is therefore evident that ethnicity has an influence in the presence or absence of blood pressure.

Most people with the diabetic condition, renal impairment, elderly or severely ill suffer from hyperkalemia, that is, high potassium in the blood. Maxzide should not be administered to patients with hyperkalemia and should discontinue immediately. There’s a likelihood that CF could be suffering from hyperkalemia following the fact that he is diabetic and elderly. That could be the reason why his blood pressure is still high despite taking the treatment. I recommend Calcium channel blockers. They are drugs that lower blood pressure by slowing calcium movement in the heart’s cells and in the walls of the blood vessels. It enables the heart to pump the blood and widen its vessels. With enlarged blood vessels, the heart needs not overwork hence the blood pressure is lowered (Liao et al., 2016). Norvasc (Amlodipine) is one of the best Calcium channel blockers used to treat high blood pressure. It is taken orally with or without food only once daily. Most people prefer taking it with water or milk. I would recommend that CF takes it with some milk of food. Dosage may be increased or decrease it depending on CF’s reaction. Calcium channel blocker is ideal for the black and elderly patients such as CF. He should avoid grape juice of fruit as it interferes with the drugs reaction on the body. I would ask CF to ensure that he eats a balanced diet and, a lot of fruits and vegetables. He should also ensure that he does lighter exercises like walking 30 minutes daily. He should also reduce his sodium intake, red meat, and cholesterol.

The US legal system places a lot of importance on eyewitness memory. Most people would report that they can accurately convey what they saw in a particular situation. However, these ideas are not supported by research. Instead, research shows that memory is quite malleable and is affected by many factors. This research repeatedly demonstrates that people do not remember exactly what they experienced. This module’s experiment will show you firsthand how memory for events is not always one hundred percent accurate.

Access the CogLab demonstration False Memory. Follow the instructions to complete the demonstration to familiarize yourself with false memory. Then locate at least one research study from a peer-reviewed journal that examined how eyewitness memory can be affected by false memories.

You are considered to be an expert in false memories, and a local district attorney has therefore requested your expertise on the following case:

On Tuesday, March 6, 2007, a bank was robbed in Slidell, LA. It was just after opening time, 9:04 a.m., and there were barely any customers, when a car arrived and parked in the side parking lot of the bank. Two men came out of the car and walked to the entrance. Both wore dark clothing. Upon entering the bank, they held out guns and asked for the manager. When the manager identified herself, the smaller of the two robbers ordered her to open the safe. Meanwhile, the other robber, a tall, and burley man, walked around holding his gun in his outstretched arm, and threatening the remaining employees and customers. The manager complied and the smaller robber collected all the money and valuables from the safe. After five minutes, the big robber asked if his companion was ready to go. When he was, the two ran back to their car, and drove away.

The district attorney has asked that you create a presentation about false memory and explain how it might influence this case. He asks that you specifically address the following:

An informational interview is a recommended tool for exploring career options. This assignment is designed to prepare you for an informational interview of a professional in the field of psychology. You will conduct this informational interview during Module 2. Here, in Module 1, you will create a script, design a list of questions, and identify the person you hope to interview. You also need to schedule the interview.

In M3 Assignment 2 LASA 1, you will submit a written account of the informational interview you conducted with the psychology professional.

You will ask the interviewee to describe the educational and training steps (the career paths) he or she took to reach the current position. You will also ask the interviewee to describe, in some detail, the job responsibilities associated with the position. The interview must be at least thirty minutes long. Ideally, it should be conducted in person. If that is not feasible, a phone interview is adequate.

For this assignment, do the following:

  • Begin to prepare for this informational interview by identifying the professional you plan to interview. Ideally, the professional should be engaged in a career that you hope to pursue. (You may want to identify a second candidate in case your initial choice is unable to complete the interview with you.) This professional may work in your local area, but that is not necessary. If you cannot identify a professional easily, ask your friends and family if they know anyone who works in the field of psychology. You may also want to phone your local community mental health center or social services agency to ask for names and contact information of potential professionals you could interview. Contact your facilitator for guidance if you have difficulty in identifying someone.
  • Provide the following identifying information about the individual you have selected:
    • The name
    • The job title (licensed psychologist, social worker, or case manager)

  • Ask the interviewee about his or her work, academic preparation, and advice. In addition, include the interviewee’s answers to questions on the following topics:
    • The experience, skills, and knowledge necessary for success in a career in psychology or in graduate school

    • His or her views on professional ethics and academic integrity
    • The role of psychological research in his or her work
    • His or her greatest challenges and rewards

  • Develop a script (word-for-word written statement) that you will use during the warm-up (meet-and-greet) portion of the interview. What will you say to the interviewee when you meet him or her? Include small talk of a professional nature that will serve as an introduction and explanation of the purpose of the interview. PART 2—Process Analysis 20%

Overview:

Process analysis is a technique that explains the steps or sequence involved in doing something which satisfies our need to learn as well as our curiosity about how the world works. There are two types of process analysis: directional and informational. Informational process analysis goes beyond recipe or formula to explain. Your purpose is not only to explain a process but also to convince the reader that the process you explain is the best way to do what you describe—this involves analysis and argument.

Instructions:

For Part 2, you will write an informational process analysis which should go beyond a simple “how to” in order to help your readers understand how something happens even though they won’t actually follow the steps outlined. You will have the dual purpose of analysis and argument. When analyzing the process, you also need to convince your audience that the process you describe is the best process to take to reach the goal of becoming a __________ in this particular career field.

You will write an informational process analysis explaining how a student of ___________ becomes a successful ______________.

For this essay, you should continue to develop a formal academic tone and use third person point of view to help establish that tone. All of your writing is based on your perspective but you must learn how to present your perspective without using “I” or “you” to do it.

Example of a thesis:

In order to become a successful radiology technician one should first complete the radiology program at North Central Texas College, then work for Advanced Medical Imaging of North Texas, and finally get promoted to Director of Operations.

*The thesis states the subject (steps to become a Rad Tech) and the point of view (specifics to be a successful one).

*Note that the thesis lists three major steps. Not every small step should be included in the thesis statement only the major points to be covered in the essay should be in the thesis.

This essay should be organized, logical, formal, and persuasive. No one wants to read an essay explaining how to be a mediocre nurse or teacher for example. Your essay should also persuade readers to follow your process to become a successful nurse or teacher (or whatever profession you’re discussing).

Part 2 Criteria:

  • Provide clear, logical, well organized informational process analysis
  • Process is explained and is convincing to the reader
  • Include a clear and focused thesis statement
  • Exhibit a formal academic tone—third person only

PART 3—Interview 20%

For Part 3, you will interview someone currently working and/or teaching in the career field. You will need to develop at least 10 interview questions for them to answer. The questions should be thought provoking and interesting and require detailed answers. The person you interview should not be related to you.

For Part 3 you should include:

  • A brief one paragraph introduction of the person (their credentials, experience, demographics)
  • Your 10 (minimum) questions and the person’s answers in Q: A: format (see example)

A: First, I study all the pieces to determine if there are any complimentary themes, colors, and sizes. I have this

space projected in my mind and I play around with different combinations trying to visualize what will work best together. When you walk in the gallery, what will be the first thing someone sees? What will the focal piece of the show? I try to visualize these things first before I even start physically moving the art around.

PART 4—Annotated bibliography 35%

Length: 4-6 pages

Instructions:

For Part 4, you will submit a detailed annotated bibliography. The annotated bibliography is the works cited page for this project—it should include ALL sources used. Each source should have a proper MLA citation, a 4-6 sentence summary, and a 2-3 sentence evaluation of the source (how useful and/or credible the source is and how it will fit into your project).

This will require a significant amount of effort and is an integral part of the research process.

AS SOON AS YOU BEGIN YOUR RESEARCH, YOU SHOULD BEGIN THE ANNOTATED BIBIOGRAPHY. This is something you work on as you go to document your research process and help you to keep track of all your sources.

Part 4 should:

  • Include at least 5 sources minimum (10 max)

Therefore, any counselor must understand how these multiple systems operate and must be knowledgeable about the programs and networks that support the mental health of diverse children and adolescents in the community.

For this assignment, you will describe how you would work with diverse individuals and two different constituencies by developing a group crisis intervention proposal for the Middle Valley Consortium. The intervention will include counseling components, education and prevention components, and follow-up plans to connect individuals to other programs or networks, if needed.

Your proposal is being requested because the community is still reeling from the shocking death of two middle school students, who overdosed on prescription drugs. Your proposal will include plans for holding two crisis intervention groups, one for the classmates of the deceased children and one for parents and community members who are upset by the event and concerned about the growing epidemic of prescription drug abuse among children and adolescents.

Your community is diverse, so your crisis groups will include members from several different racial and ethnic backgrounds. You also have been informed that there is one middle school girl who has a hearing impairment, as do her parents, and one middle school boy affected by a motor neuron disease resulting in orthopedic impairment; he requires a wheelchair. His father is also affected by this heritable disease and requires a wheelchair. Additionally, from previous experience in the school, you know that two of your group members are medicated for ADHD.

Discuss why group crisis counseling is helpful for children and adolescents who experience crises and how this meets important mental health needs. Select a counseling theory that will guide your crisis interventions and state your rationale for using it for this purpose.

Skills Required in Diverse Groups

Teaching Phase

One responsibility of counselors is to use multicultural competencies to ensure that individuals from all background benefit from our services. In group settings, this can be even more challenging due to differences among the group members. Even group members from the same race have different levels of acculturation, and so might respond to counseling strategies differently. What will you do to remain alert and responsive to needs of the group? How might you need to modify your counseling skills, techniques, and the group crisis counseling intervention to make it culturally appropriate for diverse individuals?

Assuring Appropriateness for Individuals With Exceptionalities

All individuals have the right to receive mental health services that meet their needs. The PPACA was aimed to improve the health care system, offering affordable health insurance to more Americans, expand the Medicaid program to cover all adults that live below the poverty level, and to support innovative medical care delivery methods designed to lower the costs of health care generally (Lampert, 2009). The PPACA mandates that everyone that is legal will have health coverage in the United States or a penalty will have to be paid at the end of the year. With the influx of people now receiving healthcare, they will be seeking medical attention where in the past they would not. The increase in people seeking medical attention will result in a needed increase in nurses to take care of these patients and more training to gain more expertise and skills to be able to manage the emerging needs and diseases.

A key nursing provision is the health care workforce loan repayment program (Section 5203). This section focuses more on children. It has a focus on pediatric medical or surgical and children mental and behavioral health care. The repayment program will offer psychiatric nurses, social workers, and professional/school counselors up $35,000 a year in loan repayments for their participation in an accredited pediatric specialty residency program.  A second key provision is public health workforce recruitment and retention programs (Section 5204).  This provision is aimed at eliminating the shortage of public health agencies. This provision has offered a one third repayment of loans for an exchange of a contract of employment acceptance with a public health agency for three years. This program is offered to eligible public health or health professions.

I know my nursing career will be affected by the provisions that were listed above along with all the other provisions listed in the PPACA. Offering loan repayments will give nurses the incentive to obtain higher degrees. Recruitment and retention would likely decrease the nurse shortage by new nurses coming in and showing loyalty to the season nurses.

The effect of the Patient Protection and Affordable Care Act (PPACA) resonates profoundly in the U.S. healthcare environment and among the public. Two key provisions of the act include: 1) Guaranteed issue prohibits insurers from denying coverage to individuals due to pre-existing conditions; 2) Section 5204 Public health workforce recruitment and retention programs.

With the first requirement, health care setting experienced a rise in the number of new patients, as expected. Many of these patients were probably afraid or restricted from going to the hospital or seeking medical care due to the lack of healthcare coverage. The impact is that more people are seeking medical care. From my nursing practice, I observe some of these patients have chronic illnesses that have been neglected and thus require more care. Also, more patients come for medical services; emergency rooms fill with more patients. This increases patients’ population. In return, this affects nurse to patient ratios. Nurses have to care for more patients. They have to work extra time due to shortage of staff. The downsides is that nurse morale is low and the risks for medical errors are high.

The second nursing provision (Section 5204) retained refers to the established “Public Health Workforce Loan Repayment Program.” This provision gird to assure an adequate supply of public health professionals to eliminate workforce shortages in public health agencies. The positive impact of this provision is that government will repay up to “1/3 of loans incurred by a health professions student in exchange for an agreement to accept employment with a public health agency for at least three years” (CDC). Ultimately, this provision makes it easier to attract young people to the field of nursing. This will have a positive impact on the nursing shortage. It is an attract incentive for many current nurses to pursue their studies and opt for advanced degree in nursing. This is exactly what I am intended to do.
