For this written assignment, you will demonstrate your understanding  of attribution theory. In essence, attribution theory states that  individuals tend to make sense of (logically prescribe) situations by  associating them to self, others, thoughts, feelings, or actions. This  theory suggests that learners should consider why they do what they do,  and what or who they are giving credit for both the victories and the  failures. Further, this theory suggests that if a person believes that  they are not good at something, they may attribute their unsuccessful  outcomes to external factors, rather than to themselves. In contrast, if  individuals have success, they more often may attribute their successes  to internal factors.

Using your required resources to support, discuss the following:

  • Describe a time where you feel you have failed and blamed someone  else: the teacher, the friend, a loved one. (Failure could be academic,  relational, and/or organizational – loss of a job.)

You are the Chief Risk Management Officer for a large metropolitan not-for-profit teaching hospital. You have been assigned this case by the Chief Executive Officer due to the potentially sensitive nature of the situation and the CEO’s personal interest in disadvantaged and ethnic minority population groups. Your goal is to do a thorough risk analysis and create a risk-management plan as part of the overall quality management program for this facility based on the following case scenario.

A female patient, age 25 years, presents in the emergency room with pain in the right side of her lower abdomen. Her last name is Jonesky. She has been ill and vomiting for two days. The patient is triaged and asked to take a seat in the emergency room waiting area until she can be seen. Additionally, the patient speaks only Russian, and her husband, who is the only family member with her, speaks limited English, with Russian being his primary language.

Several hours have passed and the patient is still sitting in the emergency room, in extreme pain. The husband approaches the emergency room desk to try to communicate that his wife is worsening and in extreme pain. After several minutes of discourse, the patient is taken back into an examination room to be seen.

She is diagnosed with appendicitis and requires removal of her appendix. She is sent to a holding area outside of the operating room where she will wait to be taken into surgery. Upon her arrival, a nurse asks her name and due to the fact that the patient only speaks Russian, her name, “Jonesky” sounds very much like “Jones” to the nurse. Then anesthesiologist arrives and interviews the patient. Simultaneously, another emergency patient is brought into the holding area of the operating room. Her name is Samantha Jones. Samantha has fallen down icy steps and has broken her left ankle, which now needs to be repaired. This is a very busy night for the operating room. Additional staff members, who have already worked a full eight-hour shift, are called in to attend to this second case.

Both patients are interviewed and taken into ORs for their procedures. Since it is late, both surgeons do the required time-out procedure, but not as judiciously as they would have during the day. Staff members in attendance for both cases implicitly go along with their surgeon’s order and surgery following a time-out procedure that was not complete. As the first surgical procedure begins, the surgeon makes an incision into Mrs. Jonesky’s ankle and there is no sign of any broken bone. He realizes there is a problem. Additionally, in the second OR, the surgeon operating on Samantha Jones makes an incision and finds no sign of appendicitis. Both surgeons operated on wrong sites due to incorrect identification of the patient.

The application of evidence-based principles to a public health issue demands a scientific quantification of said public health issue. An integral component of the community assessment should include the targeted health condition or risk factor being considered, the population affected, the size and scope of the problem, prevention opportunities, and potential stakeholders. To aid in this task, epidemiology and the US surveillance system are applied to track and obtain necessary information about the frequency of the health condition or risk factor in an affected population.

Although data from surveillance systems can be used to obtain baseline and follow-up measurements for target populations, there may be limitations when using the data to evaluate intervention effectiveness for narrowly defined populations. In such a case, it may be necessary to estimate the frequency of disease or other health condition for the target population by using special surveys or appropriate study designs. Understanding the tradeoffs of various study designs will improve how you evaluate the effects of various public health programs and policies.

Review the study designs from the readings assigned and suggested for this week and present a brief summary of the appropriateness and advantages of using each study design as part of your effort to quantify the public health issue that you identified in Week 1 as part of your community assessment.

Write your initial response in approximately 300 words.

By the due date assigned, post your response to the appropriate Discussion Area. Through the end of the week, review and comment on at least two peers’ responses. Complete the following:

    • Let us say that we are assisting an adolescent from Germany who has moved to United States, and is having a rough time adjusting to her new high school. In addition to information gathered from her, her parents, and the school, we could consider Erikson’s psychosocial theory and the stage of identity and role confusion in which adolescents search to establish their own values, beliefs, and goals. During this important time of identity development, the teen has been immersed in an entirely different culture with values and beliefs different from the one with which she knows. We could consider to what extent she was mastering the stage prior to and then after the move. Based on sociocultural theory, we would enquire about the differences in culture that may be impinging on her success in interacting in her new environment. By integrating the two theories, we can get a better understanding of how her developmental stage is clashing with her sociocultural context. Maria, a 70-year-old Hispanic woman, lives with her daughter, son-in-law, and granddaughter in a spacious home on the lake. She enjoys reading on the patio, walking along the river’s edge, and taking her granddaughter to the park. She is fluent in Spanish, yet understands and speaks a little English.

Angel, Maria’s daughter, has noticed some behavioral changes in her mom that are troublesome. Her mom fell while walking to the car before they left for the restaurant and she had fallen the week before when walking into living room. Sara, Maria’s granddaughter, told her mom that she had to hold Grandma’s hand and bring her home from the park because she forgot how to get home.

Angel called the doctor and scheduled an appointment for her mom. Once Maria learned her daughter was planning to take her to the doctor Friday, she was very upset because she hadn’t been consulted first. Also, Maria didn’t have any bothersome symptoms and decided there was no reason a doctor needed to see her. All she needed was more sleep.

Maria and Angel arrived at the doctor’s office early Friday morning. The nurse brought them back to the examination room upon arrival as they were the first appointment of the day. Displeased, Maria allowed the nurse to take her weight, pulse, and blood pressure. Once the nurse was finished, she left the room and the doctor came in. While asking Maria some questions, the doctor spoke slowly and loudly as if she couldn’t hear. After the doctor learned that Maria’s understanding and speaking of English was minimal, he talked to Angel the entire time and then ordered additional tests without Maria’s consent. Maria was furious and wanted to go home.

Consider the individual, family, cultural beliefs and expectations, and ethical issues the individuals must think about as they interact with each other.

Within the context of your intro, share your perspective on the importance of “finding balance” between the roles of healthcare delivery providers, such as nurses and assorted clinicians in a hospital environment next to the business/financial component that drives variables such as costs, length of stay, discharge protocols, admit/surgical denials, and any others.  Further consider that there is (or should be) a synergy among/between patient care providers and bean counters, yet it requires money to keep the beds open and salaries paid—just as it takes the care provided by trained professionals on the units.  How effective is writing “policy” on matters such as these?

Much of the ongoing debate over scope-of-practice (SoP) laws that govern the practice of nurse practitioners (NPs) across the country focuses on the cost of and access to health care and on whether these laws legitimately promote patient safety or are simply anticompetitive restrictions on NPs’ ability to compete with physicians. Discussion 4:

CS/SB 614 authorizes  an ARNP to prescribe, dispense, administer, or order any drug, which would include controlled substances.

ARNP disciplinary sanctions are added to the bill in s. 456.072, F.S., (Section 5) to mirror a physician’s sanctions for prescribing or dispensing a controlled substance other in the course of professional practice or failing to meet practice standards. Additional acts for which discipline may be taken against an ARNP relating to practicing with controlled substances that are added to the Nurse Practice Act (Section 10) include:

 Presigning blank prescription forms.

 Prescribing a Schedule II for office use.

 Prescribing, dispensing, or administering an amphetamine or sympathomimetic amine drug, except for specified conditions.

 Prescribing, dispensing, or administering certain hormones for muscle-building or athletic performance.

 Promoting or advertising a pharmacy on a prescription form unless the form also states that the prescription may be filled at the pharmacy of your choice.

 Prescribing, dispensing, or administering drugs, including controlled substances, other than in the course of his or her professional practice.

 Prescribing, dispensing, or administering a controlled substance to himself or herself.

 Prescribing, dispensing, or administering laetrile.

 Dispensing a controlled substance listed in Schedule II or Schedule III in violation of the requirements for dispensing practitioners in the Pharmacy Practice Act.

 Promoting or advertising controlled substances.
