As you read, look at how you (taking on the persona of the theorist) developed the theory. In the first stage, theorizing occurs. This is where you, as the theorist, identify the concepts of what nursing is and is not.

Perhaps you questioned what concepts were guiding those in nursing practice and then started to question your role. You started asking yourself, “Where am I in nursing, and where the profession is going? Is there some overarching concept that guides the professional in his or her practice?” This is where you recognize that a theory is needed. In the second stage, syntax is developed. This is where you will define the terms. Look for changing definitions of terms. Consider, for example, Jean Watson. She starts by defining the word “caring.” Within the last ten years she has refined her terminology changing the term caring to caritas.

This demonstrates a growth and maturation from decades of research she and other scholars did to produce the theory of caring. The third stage is theory testing. Defined as the phenomena behind the theory that are exposed through research. The definitions of terms are refined. The theorists and other researchers consider whether this theory helps answer questions that arise in practice. This is where your theory is used in by a widening group of researchers. For example, graduate nursing students request the tool you developed while testing your theory.

The fourth and last stage is evaluation. This is where philosophical debate occurs as the concepts are applied through evidenced based practice in the act of providing nursing care. Developmental psychologists study physical, cognitive, and social changes from womb to tomb.  One point of contention amongst developmentalists concerns the relative influence of nature (i.e., biological inheritance) versus nurture (i.e., environmental affordance).  In this discussion, you will consider these issues.

  • Watch Babies, which follows four babies in different geographical locations (USA, Tokyo, Namibia, and Mongolia) for the first year of their lives, documenting the vastly different worlds these children occupy.  Note the many developmental milestones that people around the world will meet regardless of cultural context.  Consider the great impact different environments might have on various aspects of development:  nutrition and physical development, childcare arrangements and social development, parenting styles and emotional development, educational practices and cognitive development, just to name a few.

Nestor is a 40-year-old black man. He comes into the walk-in free clinic at 4:45 p.m. on Friday afternoon to have a prescription for antihypertensive medication filled. A new patient at the free clinic, Nestor reports that he has had hypertension for 15 years. He says he sees his private physician approximately every three months and has been on his current medication for the past five years. He denies any current problems. When the nurse takes his blood pressure, she is surprised that it is 196/126. Her concern prompts her to question Nestor in more depth. He is in a hurry and only wants his medication. He tells the nurse that his blood pressure is always this high—sometimes higher. He says his doctor has tried many medications and found none to be more effective than the present regimen. He reports that his physician has said to him that very high blood pressure is normal for him and should not concern him because his body has adjusted. Knowing that hypertension can have devastating long-term complications, the nurse wonders whether she should disagree with this man’s primary physician. After all, Nestor did not come to the clinic for primary healthcare, but merely to have his prescription filled.

Company Biography

Amazing Coffee Houses (ACHs) is a small but established company in the coffee industry. It is based in northern Chicago, IL and employs over 100 workers locally in its 10 stores. Its current Chief Executive Officer (CEO) and Chairman of the Board, Sam Ryder, founded the company back in 1994 with his wife, Gloria Ryder. Gloria has served as the company’s Chief Financial Officer (CFO) since it was founded. Both Sam and Gloria graduated from the #1 ranked MBA program in the country and bring a lot of corporate experience to the business. Sam was a Senior Vice President (SVP) in Marketing and Sales at another company for many years before deciding to venture off to start ACHs with his wife. Gloria also worked for that same company and held the position of Finance Director.

The company is known for its own brand of gourmet coffee and low fat donuts, as well as for their own line of stainless steel coffee mugs that are able to keep drinks hot for 12 hours straight. The workforce is comprised mostly of bakers, managers, baristas, greeters, clerks, and cashiers. They also work closely with several large suppliers and distributors in the United States.

Problem

The company has a loyal customer base throughout Chicago and is known for its personalized service and quality food and drinks. Because the economic conditions have greatly improved throughout the United States and the company now has a well-established name and brands, ACHs is looking to expand in markets beyond Chicago. The first market it has made a decision to expand into is 81 miles away, Milwaukee, WI. The company has worked through a local real estate agent and was able to strike a great deal on a 10-year lease in a great location. Management thinks Milwaukee is where the next 10 stores will be opened. However, Sam and Gloria have never operated a multi-state business before and are worried they will fail to accomplish their expansion goal timely. Opportunity(Project Integration & Scope Management)

Sam has no project management experience and has decided to hire a project manager to manage the expansion project in Milwaukee, WI. After thorough evaluation and dozens of interviews, they have asked Jim Young, a rising star at the company, to take on the role of project manager to lead a team of five people from different departments to create a project plan for the new site.

Jim Young currently serves as a store manager for the biggest two coffee shops. Prior to this role, he worked as a store supervisor for the company. They will also enjoy freshly baked low-fat donuts that aren’t available anywhere in the state. ACHs will hold true to its vision and mission of providing customers with a premier café experience. After completing your stakeholder analysis and developing your stakeholder register, you started working on your next project, which will be to develop a project charter. You started to gather information from various stakeholders via interviews and e-mails. The latest e-mail you sent caused quite a response. Several meetings were centered on the project charter, statement of work (SOW), work breakdown structure (WBS), and enterprise environmental factors and organizational process assets that you will use to complete the second and most important deliverable at this stage, the project charter. Jim comes to your desk one afternoon for further discussion.

“Our team meetings on the SOW, project charter, WBS, and environmental factors and organizational process assets have been very productive,” says Jim. “Thanks for getting this project charter moving in the right direction.”

“Anytime,” you say.

“So, based on our last team meeting, do you think we are ready to write a concise SOW and the project charter?” asks Jim.

“I think we have enough information to assess risks, assumptions, define scope inclusions and exclusions, objectives, business need, milestones, high-level budget breakdown, acceptance criteria, and constraints,” you say.

Margaret Newman’s theory, Health as Expanding Consciousness, “was stimulated by concern for those for whom health as the absence of disease or disability is simply not possible” (Nursing theory, 2016). Newman’s theory “asserts that every person in every situation, no matter how disordered and hopeless it seems, is part of the universal process of expanding consciousness” (Nursing theory, 2016). Newman expressed that removing pathology will not change the pattern of the individual pattern (Nursing theory, 2016).

Newman expressed that “nurse client relationships often begin during periods of disruption, uncertainty, and unpredictability in patients’ lives” (Smith & Parker, 2015, p. 288). Majority of the time patients who are in the hospital are there due to unexpected events that occurred and now they are hospitalized. Some patients suffer such traumatic injuries and develop lifelong complications that will affect everything and everyone around them. As nurses, we tend to evaluate each and every patients situation, and at times we are more concerned and relate to the patients situation. Working in a rehabilitation hospital, I use theory very often, I would say probably on a day to day basis in my practice.

There are many stories I could share with the experiences I have had with some patients, however, I am going to share my experience I had with a patient that stayed in the hospital for about 2 months. He was a 66 year Old man, who had fallen off a tree, while trying to get the family cat from the tree, and suffered a major spinal cord injury. He was paralyzed in his lower extremities with very limited movement in his upper extremity. He spent a lot of time in an acute hospital, before he came to our facility for rehab. Each day I spent taking care of him, I learned something new about him every day. I learned about his family, what he did for a living, about his kids, and soon we developed a relationship where we would joke around and laugh and talk about his progress in therapies. He was a very sweet and kind man. He was the provider of his family, he was independent and was just not ready to give up. His wife and daughter came to visit him every day. Even though he joked around and was happy we knew that deep down inside he was suffering.

The day case management met with the family and patient about discharge planning, the wife told them she would not be able to take care of him, he needs a nursing home. The tears just rolled down the patient’s eyes. The process of discharge continued because the patient did not have the best insurance so finding placement was not very easy, but our case managers were wonderful and kept getting him more time in the rehab hospital. From nursing, to therapy, to case management, and almost every department knew his story and everyone just adored and loved him. The wife and daughter started to come visit less and less, and the one day the patient just broke down and said “I am useless, I am burden to my family, I cannot even take care of myself, I don’t blame my wife, I am nothing but a body, I just wish I would die”. I had to hold back tears. This is the person who provided for his whole family, and now that same family is unable to provide for him. As days went on he became less active, more quiet, depressed and ill. He stopped eating and drinking, he just wasn’t himself.

One day I went into change him, and we talked about therapy and the weather, my nursing tech helped me roll him over, and all of the sudden she said “I don’t think he is breathing”. His eyes rolled to the back of his head, we called a code and CPR was started and 911 was called. He had a PE, went into cardiac arrest and never came back. One of the saddest experiences of my life and probably of all the staff that had the pleasure of taking care of such a special man. As a healthcare team, we provided him with the best care, he developed relationships with all his nurses, nursing techs, therapist and doctors.

We build relationships with majority of our patients. Sometimes we are the only family these patients have. When life throws unexpected twists and turns at them, they are scared, angry and feel hopeless, and they turn to us for answers and that little bit of hope and confidence. I can say with confident that this patient knew and trusted us, even though he knew he would not be the same as before.
