What are the ethical and legal implications?

(What are the ethical and legal implications?)

Question description

Question 1: Fred is an 83-year-old male who is being admitted to the medical-surgical unit status post fall. He is alert and oriented and reports that while visiting a local casino with his wife Margaret earlier this evening, he tripped over a curb and fell landing on his right side. After receiving morphine in the emergency room prior to transfer to your unit, Fred is rating his pain at 6/10. He has multiple bruises from his jawbone to his knee as well as a slight rotation of his right leg.

Past medical history includes: myocardial infarction (MI) x 2, peripheral vascular disease (PVD) with bilateral iliac stents, non-insulin-dependent diabetes mellitus (NIDDM), sleep apnea, and degenerative joint disease.

Medications include: aspirin, Plavix, Lopressor, Lisinopril, and Metformin.

After reviewing the above scenario please answer the following questions.

  1. Based on the information provided, how will you prioritize your care, what assessments will you include and in what order? Please provide rationale for your response.
  2. Considering this patient’s age, injury, past medical history, and list of current medications, what, if any, concerns do you have related to his potential need for surgery?
  3. Should surgery to repair his right femur be required; what type of clearance and pre-op orders would you anticipate receiving related to his diet, meds, lab work, and so on?

Question 2: This week, we will discuss personal communication devices and their use in healthcare. As we focus on this topic, please address the questions below in the discussion.

  • How can the use of the nurse’s personal communication device(s) impact patient care positively and/or negatively?
  • What are the ethical and legal implications?
  • What does the professional literature say about how communication devices can support safe nursing practice?

Question 3: Each academic area of study has a particular style for documenting the ideas of other scholars. The standards of formatting document you’ve been using is from the American Psychological Association (APA). This is the preferred style in the social sciences. As you’ve noticed, there is a strong emphasis on the publication date. Why do you think there is such an emphasis on the date? As you work on revising your Course Project, what questions do you have about how to incorporate standard APA format and documentation guidelines into your essay? Are there any sources you are having a hard time documenting? Any questions about your reference entries?

PUBLIC HEALTH:TYPE 2 DIABETES

PUBLIC HEALTH MY HEALTH ISSUE IS TYPE 2 DIABETES PLEASE FOLLOW DIRECTIONS

(PUBLIC HEALTH:TYPE 2 DIABETES)

Based on the readings assigned and suggested for this week and a review of the seven-stage, sequential framework to promote greater use of EBPH, do the following:

The application of evidence-based principles to a public health issue demands a scientific quantification of said public health issue. An integral component of the community assessment should include the targeted health condition or risk factor being considered, the population affected, the size and scope of the problem, prevention opportunities, and potential stakeholders. To aid in this task, epidemiology and the US surveillance system are applied to track and obtain necessary information about the frequency of the health condition or risk factor in an affected population.

Although data from surveillance systems can be used to obtain baseline and follow-up measurements for target populations, there may be limitations when using the data to evaluate intervention effectiveness for narrowly defined populations. In such a case, it may be necessary to estimate the frequency of disease or other health condition for the target population by using special surveys or appropriate study designs. Understanding the tradeoffs of various study designs will improve how you evaluate the effects of various public health programs and policies.

Review the study designs from the readings assigned and suggested for this week and present a brief summary of the appropriateness and advantages of using each study design as part of your effort to quantify the public health issue that you identified in Week 1 as part of your community assessment.(PUBLIC HEALTH:TYPE 2 DIABETES)

Write your initial response in approximately 300 words.

By the due date assigned, post your response to the appropriate Discussion Area. Through the end of the week, review and comment on at least two peers’ responses. Consider commenting on their stated advantages of using a specific study design. Do you agree or disagree with their stated advantages and why?

Support your answers with examples and research. Your responses should clarify your understanding of the topic. They should be your own, original, and free from plagiarism. Follow the APA format for writing style, spelling and grammar, and citation of sources.

Evaluation Criteria:

  • Summarized the appropriateness and advantages of using each study design as part of the effort to quantify a public health issue as part of the community assessment.
  • Responses demonstrated in-depth research and analysis.

Immune Disorders In Pediatric And Geriatric

Immune Disorders In Pediatric And Geriatric

(Immune Disorders In Pediatric And Geriatric)

MSN degree APA format 3-5 pages 2 References from Walden University Library

Due 3/16/18 7pm EST

Immune Disorders in Pediatrics and Geriatrics

In this week’s Discussion, you began exploring the pathophysiology and treatment for immune disorders, such as HIV, psoriasis, inflammatory bowel disease, and systemic lupus E. You continue this exploration as you consider these disorders in specific patient groups: pediatrics and geriatrics. As an advanced practice nurse, you must understand the differences in the pathophysiology of these patients, as well as how the drugs used to treat immune disorders might impact the body systems of pediatric and geriatric patients. These two patient groups process and respond to drugs differently than the average adult in their absorption, distribution, metabolism, and excretion of drugs. For this reason, special consideration must be taken when treating immune disorders in pediatric and geriatric patients.

To prepare:

  • Review this week’s media presentation on antimicrobials.
  • Select one of the following immune disorders: HIV, psoriasis, inflammatory bowel disease, or systemic lupus E (SLE).

Note: Select a different disorder than the two that you selected for this week’s Discussion.

  • Think about the pathophysiology of your selected disorder. Compare pathophysiological changes in pediatric and geriatric patients.
  • Consider the types of drugs that are used to treat symptoms associated with your selected disorder. Reflect on how the drugs might impact various body systems in pediatric and geriatric patients, as well as measures you might take to help reduce any negative effects.
  • Reflect on how you might educate a pediatric patient on the disorder you selected and how you might educate a geriatric patient on the same disorder. Consider how you would promote medication adherence, as well.

To complete:

Write a 3- to 4-page paper that addresses the following:

  • Explain the pathophysiology of the disorder you selected. Then, compare pathophysiological changes in pediatric and geriatric patients.
  • Describe the types of drugs that are used to treat symptoms associated with the disorder you selected. Explain how these drugs might impact various body systems in pediatric and geriatric patients, as well as measures you might take to help reduce any negative effects.
  • Explain how you might educate a pediatric patient on the disorder you selected and how you might educate a geriatric patient on the same disorder. Include your rationale for each approach, as well as how you would promote medication adherence.

Media Violence: Protecting Children

Media Violence: Protecting Children

(Media Violence: Protecting Children)

Go watch some TV this week!  Yes, for homework!  Specifically, you are going to watch an hour of children’s shows (preferably cartoons) and analyze the violence you see.  We will be discussing your analysis in the discussion.

The assignment this week is the first LASA.  For this assignment you are in the role of a behavioral consultant, and will be giving advice to concerned parents about the level of their son’s exposure to media.  There are many components to this paper, so make sure you address each one.  I would recommend structuring your paper with subheadings, which you get from the grading rubric (e.g., “Prevalence” as the first subheading, “Dangers of media violence” as the second subheading, etc.).  If you structure your paper this way, you will not inadvertently skip over any sections, and it is a wonderful way to organize your paper.

******Protecting Children From Media

For his 10th birthday, Greg was given a handheld videogame system. His parents allowed him to pick any two games. They knew the games might contain violence, because there was a violence rating sign posted on the games, but rationalized that they were only games and other kids play them. Greg would quickly finish dinner and run up to his room to play his games.

His parents were pleased to observe that Greg enjoyed their present so much. After a week, his parents noticed that he wasn’t turning off the videogames at bedtime, and had begun turning homework in late. He was up so late playing that he would not get up for school without argument. His parents decided that enough was enough and took the video games away. Greg threw temper tantrums and persuaded his parents to buy him a computer after convincing them that it was necessary to keep up at school. Greg soon returned to the same pattern of behavior where he spent long hours and late nights at the computer. This time, his parents felt a false sense of security that he was doing his homework. One day, his curious parents decided to scan the computer history to see what Web sites Greg was browsing. To their horror, they discovered that Greg was spending many hours online playing interactive, sometimes violently graphic, games on the Internet. He was also chatting with other “gamers”. Before confronting Greg about his behavior, his mother and father agreed to investigate what types of intervention strategies might be available within their community. They have come to you, a behavioral consultant, for advice.

Click to Read the Kaiser Family Foundation Study: Generation M2. Media in the Lives of 8- to 18- Year Olds.

Describe the issue of exposure to videogame violence in today’s society as related to Greg’s situation. Explore issues such as:

  1. Prevalence (e.g., age, gender, racial diversity, etc.)
  2. Given Greg’s developmental level, what are possible dangers of exposure to media violence? Be sure to address this in the context of his cognitive and socioemotional development.
  3. Risk and protective factors, including predisposition to violence (e.g., are all children who play violent videogames likely to become more aggressive? What protective factors might mitigate the possible outcomes for Greg?
  4. Discuss the possible outcomes if Greg’s behavior continues unchecked.
  5. Discuss types of intervention strategies you would expect his parents to find at a community level, such as in community centers, schools, and social service agencies, to assist children like Greg who are at risk due to ongoing exposure to media violence.
  6. What would be a good plan to recommend to Greg’s parents?
  7. Compare the dangers of exposure to videogame violence with other forms of violence. What are similarities and differences between videogame violence exposure and the other type of violence you chose to compare?
  8. What are the costs of videogame violence to the family and the community and society at large and the other form of violence you chose for comparison?

Organizational Culture and Values

Introduction

Organizations rely on managers and leaders to fulfill their mission today and their vision of the future. The focus for this week will be on the roles of nurse managers and leaders in health care organizations, theories that underlie the practice of management and leadership, as well as the use of power in an organization.

Role of the Manager

The role of the manager is to ensure that the mission of the organization, which focuses on providing excellent care for clients, is fulfilled through the effective and efficient coordination of resources. Managers are responsible and accountable for ensuring that competent staff are provided with the tools and processes required to accomplish the work. To perform this role, managers are given the authority to make decisions that directly influence these factors within their scope of responsibility.

The functions of the management role include planning, organizing, staffing, directing, and controlling (Marquis and Huston, 2009). Within each of these functions, decisions must be made to optimize the care provided while maintaining fiscal responsibility. Planning is required to determine the best ways to fulfill the organization’s mission. As in client care, this step includes assessing the current situation and identifying actual or potential issues. Organizing provides the framework within which care is provided. Staffing refers to determining both the overall number and skill mix (e.g., numbers of licensed and unlicensed personnel required to care for a specific client population) needed as well as ensuring adequate shift-to-shift staffing. Directing includes assuring the work is being accomplished, whereas controlling encompasses both quality management and adherence to the budget. In this course, elements of each of these functions will be explored.(Leadership and management discussion 4)

Role of the Leader

The role of the manager is often viewed as one that works to maintain the status quo and ensure smooth day-to-day operations which are critical to the organization. A leader is viewed as one who encourages growth in the organization. The word itself implies movement and there is no need for a leader to simply get people to where they already are.

Leadership remains a vague concept, but ideas about what makes a great leader abound. Warren Bennis offers this definition “Leadership is a function of knowing yourself, having a vision that is well communicated, building trust among colleagues, and taking effective action to realize your own leadership potential” (The Teal Trust, n.d.). Throughout the definitions and discussions of leadership, two major themes emerge: 1) leaders are responsible for promoting growth, and 2) leaders work by influencing and empowering others.

Theories regarding leadership styles and their application also abound. Early in the discussion of leadership, three basic styles were defined. These classic types include authoritarian, democratic, and laissez-faire. Further exploration of these styles led to a theory of situational leadership, where the leader alters their approach based on the issues and people involved. Knowing the advantages and disadvantages of each approach and matching these to the situation should achieve better outcomes.(Leadership and management discussion 4)

Current theories of leadership focus on recognition and empowerment of individuals. The challenge lies in developing a culture that fosters these collegial relationships and in preparing individuals for leadership. Not everyone in a leader’s circle of influence will be interested in becoming a leader. Some find it difficult, due to their culture or experiences, to develop a different relationship with persons viewed as authority figures (Marquis and Huston, 2009). Despite the challenges, this approach to leadership of professional staff shows promise in creating a work environment that enhances nurse satisfaction (Kerfoot, 2004).(Leadership and management discussion 4)

Power and Politics

Power is defined as “the ability or capacity to act or perform effectively” (The American Heritage Dictionary, 1985, p. 971). Power is also linked to the ability to influence or control. Power is strongly linked to the roles of manager and leader since both, by definition, must be able to influence or control others in order to accomplish the mission of the organization.

People within the organization gain power in different ways. Managers are given legitimate power, otherwise known as authority, to provide rewards and consequences for staff behavior. It is well known that being given authority does not always lead to the power to manage others, as seen when staff choose not to follow policies. Bridging the gap between authority and power often requires the skill of a leader to influence people to work together and be willing to follow policies for the good of the organization or speak up when they believe the policy no longer benefits the organization. Leaders may or may not have legitimate power or authority. Leaders in this position often influence others through expert and referent power. This type of leader may be a member of the staff who is recognized as a clinical expert or who is aligned with persons in authority.(Leadership and management discussion 4)

Marquis and Huston (2009) define politics as the effective use of power. Politics recognizes that all people are interdependent; no one can accomplish the work of the organization in isolation. Learning the political climate of an organization is a key to success as a manager or leader. Developing relationships with others in the organization is a critical component. This is best done in face-to-face encounters rather than by telephone or e-mail. Possessing information is also a form of power, knowing when and with who to share that information is also important. A new manager or leader will often benefit from finding a mentor who understands the political climate of the organization.

Integrating the Roles of Manager and Leader

Not all leaders are managers and not all managers are leaders. The theories of transactional and transformational leadership highlight this quite well. A transactional leader is interested in maintaining the status quo. A transformational leader is interested in promoting growth, both for themselves and for others. Although a manager is given legitimate power simply by the authority delegated to the position, this is often not sufficient for truly carrying out the mission and vision of the organization. Conversely, leadership alone, without attention to day-to-day organization, is likely to lead to chaos. Finding a balance of the roles that suit the organization and the people being managed or led is the challenge.(Leadership and management discussion 4)

Conclusion

Although managers and leaders have distinct roles within an organization, the most effective people will blend the functions and roles in their work. Both managers and leaders need to develop a power base and use that power wisely within the organization to further the mission and goals. When management is effective and efficient, and leadership is characterized by vision, communication, and empowerment, then both the organization and the clients served will benefit.

Psychopathology and Diagnosis

Discussion 1: Impact of Mainstream Focus on Psychopathology and Diagnosis

(Psychopathology and Diagnosis)

Diagnosis and the description of mental health issues have recently become ubiquitous in the media. One of the reasons for this new emergence of the topic of mental health in the mainstream media is because of worldwide traumatic events. Many tragic events covered in the media are centered on one or two individuals who have committed acts of violence. Unfortunately, most of these individuals have had a history of mental illness. This aspect of the tragedy then becomes the focus of analysis within the media. From this point of reference, reporters and journalists educate the public on mental health issues. However, these tragedies could be an opportunity to educate the public about the facts related to mental illness. Unfortunately, the media discussions about mental illness often result in attaching a stigma to mental illness and to the population suffering from mental illness.

As a clinical social worker, you need to be aware of the impact of the entry of psychopathology and diagnosis in mainstream public discourses. You also need to think about how the media could help eliminate or mitigate the stigma attached to mental illness in the mind of the general population.

·      Post an explanation of your thoughts on how psychopathology and diagnosis have entered mainstream public discourse.

·      Then explain the potential negative and positive impact of this mainstream discourse on those living with a psychiatric diagnosis.

·      Briefly explain the DSM-5’s organization and its dimensional approach to diagnosing and its possible impact on society’s view of mental disorders.

·      Be sure to include specific examples to the mainstream media (e.g., television or magazine reports, television shows) in your post.

Support your post with specific references to the resources. Be sure to provide full APA citations for your references.

Discussion 2:
Historical Divides and Ethical Obligations Within Social Work

The National Association of Social Workers (NASW) specifies the ethics and values of the profession in their Code of Ethics. Section 6.04 of the Code of Ethics (1999) states:

Social workers should engage in social and political action that seeks to ensure that all people have equal access to the resources, employment, services, and opportunities they require to meet their basic human needs and to develop fully. Social workers should be aware of the impact of the political arena on practice and should advocate for changes in policy and legislation to improve social conditions in order to meet basic human needs and promote social justice.(Preamble, p. 8)

With the requirement of social and political action among social workers, there is little political activity among clinical social workers. Why? What barriers exist that prevent social workers from fulfilling this ethical obligation?

What does it mean to be a macro social worker? A micro social worker? Do these differentiations have any real meaning? If not, why is this language used when referring to the social work profession?

In this Discussion, you will look at the obligation of social workers to engage in political action in their practice and discuss why there appear to be different perceptions of the responsibility for political action among social work professionals.

·      Post an analysis of historical divides (such as the schism between Jane Addams and Mary Richmond) and historical influences on current social work practice with respect to policy advocacy and action.

·      Do such schisms exist in contemporary social work?

·      If you think these divides exist, how do they prevent social workers from fulfilling their ethical obligation(s)?

·      Are they important differentiations?

Case Study

C.S. was a 49 year old female with severe peripheral arterial disease (PAD) who had underwent numerous operations to reestablish circulation in her legs. I was taking care of her every few months or so when her stent would clot off or she would need another bypass in her legs. She took her Plavix and aspirin daily and could not understand why she was continuing to have problems.

Clopidogrel (Plavix) is a prodrug, meaning is must be converted by the liver into its active form using the cytochrome P-450 enzyme system (Arcangelo, Peterson, Wilbur & Reinhold, 2017).  The most active enzyme in the metabolism of this drug is the CYP2C19 enzyme (Polasek, 2011). After conversion, it works to decrease platelet activation and subsequent aggregation and is commonly used in stroke, cardiac, and PAD patients. The binding of the active metabolite is not reversible and continues for the lifespan of the platelet (7-10 days), therefore it is important to stop the drug 7-10 days before procedures like surgery (Drugs.com, 2012).

Pharmacogenetics is the study of how an individual’s genotype effects how they respond to the drug (Arcangelo, Peterson, Wilbur & Reinhold, 2017).  As a drug that relies on the body to produce the active metabolite, Clopridogrel depends upon a “healthy” or un-mutated CYP2C19 enzyme. Some populations carry an allele that decreases or even stops the activity of the CYP2C19 enzyme and subsequently makes drugs like Clopidogrel much less effective (Han et al., 2015).  This can account for up to a 70% variability in the response to Clopidogrel and may be the reason C. S. is experiencing repeated stent thrombosis (Polasek, 2011). The availability of genetic testing can help practitioners individualize drug regimens to optimize care for patients like C.S. (Scott, 2012).

Plan of Care(Need Response To Below Posting)

For C.S., genetic testing might benefit her if it is available and affordable on her insurance plan. I would see if her family history includes PAD and if her family had issues with stent thrombosis as well. If genetic testing is not an option, changing her medication regime to something that does not rely on the CYP2C19 enzyme, such as ticagrelor (Brilinta), would be recommended. With any platelet inhibitor, she must be educated on the risk of bleeding and bleeding precautions for home. To reduce the risk of drug-herb interactions on bleeding time, I would screen her home med list for over the counter supplements such as Omega-3, garlic, ginger, etc. that may interact with the medication and increase bleeding risk (Arcangelo, Peterson, Wilbur & Reinhold, 2017).

comprehensive care plan

create a comprehensive care plan for acute/chronic care, health and medicine homework help

(comprehensive care plan)

Question description

Please use the case study of heart attack upload, to provided case studies that focused on cardiovascular disorders. You will pick one of these cases to analyze and create a comprehensive care plan for acute/chronic care, disease prevention, and health promotion for that patient and disorder. Your care plan should be based on current best practices and supported with citations from current literature, such as systematic reviews, published practice guidelines, standards of care from specialty organizations, and other research based resources that are no later than 5 years old. In addition, you will provide a detailed scientific rationale that justifies the inclusion of this evidence in your plan. Your paper should adhere to APA format for title page, headings, citations, and references. The paper should be no more than 10 pages typed excluding title page and references. Some critical point to include in the case analyze are:

1. The patient just had a heart attack and he was treated for it emergently in the ER/hospital.  Many of you need to recall that when a patient is sent to his primary care giver after such an event, a thorough review of the system that is in trouble is completed.

So you should mostly be focusing on the cardiac issues.  That is why the ER doc sent him your way – for long term management of the heart.  The ER doc did not review all of his medications and place him on everything that he needs to be on – that is for you to do and determine.

So that is my first focus – how does his heart look?  well, look at the blood pressure – it is horrible.  So his tenormin is no good and either needs to be changed or increased or something added.

2. Recent history – he had stents placed.  He is only on baby aspirin -That is no longer the standard of care with stent placement. This guy needs to be on clopidogrel or some such blood thinner to avoid complications with his stents forming clots.  That is a lifetime deal – so he needs education about the medication changes, why you are changing them, what they are and how to take them.  He will need some good followup care instructions, and he will need to be strongly advised against stopping the medications for any reason. I would also explore his ability to get the medications and have a steady supply because he might have issues and maybe that is why he is reluctant to maintain them.

4. Then discuss some of his other issues in the overall picture of heart problems – possibly his weight – which at 220 pounds and 5’10” he is not too horribly overweight although his BMI ranks him at obese – just barely.  He could stand to lose 20-30 lbs. largely because he is android obese – tends to hold the excess weight around his midline.

5.  Diabetes needs to be regularly monitored and I would review this with him.  His hgA1C is 7.5 which likely means he averages blood sugar counts of between 150-175.  He may need to revert to insulin to control his sugars if he is not able to control diet enough to allow his glucophage to work. The glucophage will not do all the work – he has to help.

  • Demonstrated an evidence-based approach to address to the above key issues identified in the case study.
  • Please use the uploaded the case study to create the care plan.

Criteria:(comprehensive care plan)

  • Case Study Evaluation
    • Analyze the disorder addressing the following elements: pathophysiology, signs/symptoms, progression trajectory, diagnostic testing, and treatment options.
    • Differentiate the disorder from normal development.
    • Discuss the physical and psychological demands the disorder places on the patient and family.
    • Explain the key concepts that must be shared with the patient and family to achieve optimal disorder management and outcomes.
    • Identify key interdisciplinary team personnel needed and how this team will provide care to achieve optimal disorder management and outcomes.
    • Interpret facilitators and barriers to optimal disorder management and outcomes.
    • Describe strategies to overcome the identified barriers.
  • Care Plan Synthesis
    • Design a comprehensive and holistic recognition and planning for the disorder.
    • Address how the patient’s socio-cultural background can potentially impact optimal management and outcomes.
    • Demonstrate an evidence-based approach to address key issues identified in the case study.
    • Formulate a comprehensive but tailored approach to disorder management.
