Characteristics of Nursing Research Utilization and Evidence-Based Practice

Introduction

Research utilization and evidence-based practice (EBP) are regularly used within the nursing profession in developing positive changes for patient outcomes. Although both research methods provide results beneficial to the nursing community, each method approaches nursing research in a different manner. Research utilization focuses on implementation of the results after a study is conducted whereas EBP incorporates the research directly into the clinical decision-making process (Polit & Beck, 2006).

Characteristics of Research Utilization

In the previous lessons, the focus has been on the language and components of research. In this lesson, the focus is on how to apply research findings to improve patient outcomes. Over the past 40 years, the concept of research utilization has been described in the literature. In the nursing community, the long-held tradition has been to utilize research, through findings, to impact health changes within the profession. In order for this process to be successfully implemented into application, the research analysis would be compounded upon multiple studies within a specific nursing area. Furthermore, through the presentation of correlated findings and possibly similar results, health care professionals will have an accurate knowledge in order to influence patient and system outcomes.

Over time, issues have been raised about research utilization being affected. The key to research utilization is not only the acknowledgment and review of the findings but the implementation of those results into practice. In order for this to occur, nurses need to be active participants in evaluating current research and utilizing the results within their normal practice (Polit & Beck, 2006).

The majority of nurses in the United States are initially educated at the Associate Degree level. In these programs, the concepts of nursing research are often not discussed. Similarly, most registered nurses are not taught how to evaluate advanced nursing science, or how such advances will improve outcomes. Research utilization and evidenced-based practice principles put emphasis on these skills.

Evidence-Based Practice

Recently, a movement has been made toward evidence-based practice (EBP) research. The purpose behind this method is to implement a solution to an evidence-based problem. To accomplish this goal, registered nurses must become experts in not only reading research articles, but also in collecting relevant research findings to help them make clinical decisions. After the fundamental information is collected about a health care topic, the researcher can utilize it in creating the design and framework for the necessary research. In essence, the search for the best possible information from top-quality research, which is integrated with clinical expertise, available resources, and the needs and desires of the patient, is the basis for EBP in nursing.

The skills associated with EBP research place emphasis on diagnosis, therapy, etiology, prognosis, or prevention. EBP has been developed to provide a method for practicing nurses to understand research in a way that allows them to incorporate it into improved patient care. One skill that is helpful is the PICO method, which allows practitioners to formulate research questions. By formulating a PICO question, nurses are making the first step in preparing for their capstone project.

PICO

PICO is a mnemonic used to describe the four elements of a good clinical foreground question: (P) patient, (I) intervention, (C) comparison, and (O) outcome (see Table 5.1). The purpose of the PICO format is to assist clinicians in formulating clinical questions. This beginning process can be a challenge, but using the PICO format allows researchers to critically consider all of the components their research will address.

When developing a PICO question, researchers must take the time to carefully formulate it. They need to make it a topic about which they are passionate, and one that has a body of literature to support the intended outcome.

Conclusion

Nurses’ responsibilities lie in being observant and curious, participating in quality-management activities of their units, and supporting research activities developed by others. Using expertise and professional judgment, along with quality research evidence, is essential in developing quality care and outcomes for patients.

You and your testing team have been tasked with testing a new benefits application that is being developed. Your company uses a flexible benefits plan, allowing company employees to customer their heal benefit plan based on personal preferences. The benefits application is expected to go live in 3 months.

Your company’s Human Resources department uses a cafeteria-style approach to provide employees with health care benefits. Each September, employees review their current benefits, modify their elections and then sign election forms. Any changes must be entered and tracked in the benefits application. For medical insurance, an employee enrolls with a preferred provider organization (PPO) or health maintenance organization (HMO). The company currently pays the monthly medical premiums for its employees. However, if an employee enrolls in a dental plan or vision plan, the employee pays a modest monthly premium for the optional plan. An employee may carry medical, dental, and/or vision coverage for one or more of his or her family members. In order to carry insurance on a family member, the employee must carry the same coverage. For example, to carry dental insurance on a spouse, the employee must also carry dental insurance on him or herself.

The benefits application must track employee information like employee ID, department ID, name, address information and office extension number. It must also track information about each insurance company to include the company number and company name. Dependent information and their relationship to the insured employee must also be tracked. An employee must be able to request an enrollment form, and Human Resources must be able to update the effective date of the health benefits on the form. In addition, the application must track the health care benefits rates for PPO and HMO, plus dental and vision rates. Benefits rates differ by employee only, employee + spouse, employee + spouse + one child, and employee + family.

Therefore, it is important that the speaker notes are concise and detailed when explaining the bullet points.

◦It is recommended that PowerPoint Slides contain no more than five bullet points and should not contain more than 5 – 7 words each. Do not type paragraphs or long sentences on the slide. The information that explains each bullet point is conveyed via speaker notes or by recording your voice to each slide.

In the Final Project for this course, you are to develop a reflective Ethical Autobiography, intended to define your ethical perspective as an emerging counselor in training.

In order to understand clearly where you are headed, you must also evaluate where you have been and what has influenced you along the way. It is important to reflect critically upon your own values (and sense of personal/professional ethics) and how you developed these perspectives in order to develop an ethical framework. To help accomplish this goal, the Final Project for this course is an Ethical Autobiography in which you will explore various elements of your life experiences that might influence your future ethical framework.

As you reflect on your journey through this class, some of the course readings may have informed your Ethical Autobiography. You can also make use of outside resources, but much of the paper will be exploring what you bring to the profession and events that may have influenced your ethical lens. This reflective autobiography should have personal meaning to you and should help you understand what being an ethical practitioner means. In this sense, you are writing an intellectual and Ethical Autobiography, that is, who you are as virtue of what you believe, what you do, and what you have read. Think broadly—there are no wrong answers; you are exploring your own world.

  • What are some of your basic values that guide your work and your life? What experiences have potentially influenced your decision making?
  • What aspects of your personality and work ethic are most compatible with the counseling field? Which aspects are the least compatible?
  • Was there a time, in your personal or professional life, when you felt that your confidentiality was violated, that you were involved in a dual relationship in which you felt uncomfortable, or perhaps an issue resonated unexpectedly with you (e.g., transference)?

Essential Elements (You must address the points outlined below in your Final Project.):

  • Select four counseling issues, describe these issues, and explain potential ethical challenges for addressing these issues in your professional practice.
  • Explain state or region laws or statutes that might apply to these ethical challenges.
  • Reference specific codes of ethics that you ascribe to for your practice and how adhering to ethics and law present challenges for addressing these issues you selected.
  • Explain why this Assignment is meaningful to you.
  • Describe how adhering to ethics and law for professional counseling practice might influence social change.
  • Finally, explain how you have changed because of your work in this class. Describe personal and ethical values you have reexamined because of your work in this course.

You should present your Final Project as a 12- to 15-page (including cover page, abstract, and references—therefore, approximately 10–12 pages of text), double-spaced, APA-formatted paper. Papers can be longer if the purpose of the paper is served, but the quality of ideas and conciseness of the writing should justify the extra length. Also, please proofread your papers to make sure that grammar, punctuation, and other mistakes do not hinder the communication of your ideas.

Your Instructor will evaluate your Final Project according to all four indicators in the  Final Project Rubric, located in the Course Information area.

Furthermore, you will be evaluated on your ability to:

  • Develop your ability to think about, use, and integrate what you have read. You should go beyond just a summary of what you have read. You should also be well balanced in your arguments, and go beyond the obvious (e.g., “we should be aware of ethical issues and follow the ACA Code”) into specifics, gray areas, and more subtle questions.
  • Demonstrate that you have learned something from this course on ethical decision making.
  • Clearly communicate your reflective thinking.

The purpose of this Assignment is to explore how a theorist explicates his or her philosophy and thoughts behind a theoretical field. As in other fields of study, nursing has a plethora of theorists and theories. These theorists have developed their paradigm over time enriching it with research and dialogue with other theorists.

Directions

You are going to explore how a theorist of your choice created his or her theory. Did it develop as an acorn becoming a mighty oak over the decades? Did it arrive as a burst of light like the big bang as some believe created our universe? Was it developed from a blueprint like a spaceship where thousands of scholars worked together to create a rocket to the moon? In point of fact, how is a theory developed? This is what we are exploring.

You will pick a theorist who interests you, it does not have to be a nurse theorist. You may also choose from other disciplines such as: behavioral, leadership, business, education, technology.

You will become the theorist immersing yourself in the writings from the earliest mentioned to the most current.

As you read, look at how you (taking on the persona of the theorist) developed the theory.

In the first stage, theorizing occurs. This is where you, as the theorist, identify the concepts of what nursing is and is not. Perhaps you questioned what concepts were guiding those in nursing practice and then started to question your role. You started asking yourself, “Where am I in nursing, and where the profession is going? Is there some overarching concept that guides the professional in his or her practice?” This is where you recognize that a theory is needed.

In the second stage, syntax is developed. This is where you will define the terms. Look for changing definitions of terms. Consider, for example, Jean Watson. She starts by defining the word “caring.” Within the last ten years she has refined her terminology changing the term caring to caritas. This demonstrates a growth and maturation from decades of research she and other scholars did to produce the theory of caring.

The third stage is theory testing. Defined as the phenomena behind the theory that are exposed through research. The definitions of terms are refined. The theorists and other researchers consider whether this theory helps answer questions that arise in practice. This is where your theory is used in by a widening group of researchers. For example, graduate nursing students request the tool you developed while testing your theory.

The fourth and last stage is evaluation. This is where philosophical debate occurs as the concepts are applied through evidenced based practice in the act of providing nursing care.

Assignment Details

For this Assignment, you are going to write an article for a nursing journal explaining how you developed your theory through the four stages (theorizing, syntax, theory testing, and evaluation). Your paper must be 3 to 5 pages, not including the title and reference pages.
